【摘要】 目的 探讨垂直短瘢痕切口联合双 C 型腺体塑型技术矫正乳房肥大下垂的临床效果。本研究旨在优化切口设计与腺体塑型方法，提升手术效果及患者满意度。方法 40 例中重度乳房肥大下垂患者(年龄 28～42 岁，平均 34 岁)。根据患者的手术意愿，分为试验组(20 例)：采用垂直短瘢痕切口(长度 5～12cm，据组织切除量调整)联合双 C 型腺体塑型技术进行乳房下垂矫正术；对照组(20 例)：采用倒 T 形瘢痕法缩乳术进行乳房下垂矫正术。比较两种方法的手术效果以及患者满意度。结果 术后 1 个月，试验组 20 例患者均达到一期愈合，无相关并发症发生。对照组患者中出现 2 例切口愈合不良，通过换药后达到二期愈合。对照组中未发生术后并发症。术后 12 个月，对照组患者双侧乳房瘢痕长度明显长于试验组，瘢痕评分高于试验组，差异有统计学意义(P<0.05)。试验组患者双侧乳房对称率90.0%高于对照组85.0%，但差异无统计学意义(P>0.05)。对照组患者乳头感觉异常率达到 30%，明显高于试验组，差异具有统计学意义(P<0.05)，对照组患者满意度为 60%，试验组患者满意度明显高于对照组，差异具有统计学意义(P<0.05)。结论 垂直短瘢痕切口联合双“C”腺体塑型技术矫正乳房肥大下垂具有以下优势：操作便捷：简化手术流程；效果可靠：有效去除多余组织，重塑自然乳房形态；瘢痕隐蔽：显著缩短切口长度，提升美观度；满意度高；适用于中重度患者，短期疗效明确。更多还原

【Abstract】 Objective To investigate the clinical effect of vertical short scar incision combined with double C-type gland shaping technique in the correction of breast hypertrophy and ptosis.This study aims to optimize the incision design and gland shaping method to improve the surgical effect and patient satisfaction.Methods Forty patients with moderate to severe breast hypertrophy and ptosis (aged 28-42 years,mean 34 years) were enrolled.The patients were divided into the experimental group (20 cases) according to their surgical wishes.The experimental group was treated with vertical short scar incision (5-12cm in length,adjusted according to the amount of tissue resection) combined with double C-type gland molding technique for breast ptosis correction.In the control group (20 cases),inverted T-shaped scar method was used for breast ptosis correction.The surgical effects and patient satisfaction were compared between the two methods.Results One month after operation,20 patients in the experimental group achieved primary healing,and no related complications occurred.There were 2 cases of poor wound healing in the control group,and secondary healing was achieved after dressing change.No postoperative complications occurred in the control group.At 12 months after surgery,the length of scars on both breasts in the control group was significantly longer than that in the experimental group,and the scar score was higher than that in the experimental group,with statistical significance (P<0.05).The symmetrical rate of bilateral breasts in the experimental group was 90.0% higher than that in the control group (85.0%),but the difference was not statistically significant (P>0.05).The abnormal nipple sensation rate of the control group patients reached 30%,significantly higher than that of the experimental group,and the difference was statistically significant (P<0.05).The satisfaction rate of the control group patients was 60%,and the satisfaction rate of the experimental group patients was significantly higher than that of the control group,and the difference was statistically significant (P<0.05).Conclusions The vertical short scar incision combined with double "C" gland molding technique has the following advantages in the correction of breast hypertrophy and ptosis:convenient operation:simplified surgical process;The effect is reliable:the redundant tissue is effectively removed and the natural breast shape is reconstructed.Scar concealment:significantly shorten the length of incision and improve the aesthetics;High satisfaction;It is suitable for moderate to severe patients,and the short-term efficacy is clear.更多还原
