甲泼尼龙给药途径与合并糖尿病的突发性耳聋疗效的相关性研究
Study on the Correlation Between Administration Routes of Methylprednisolone and Therapeutic Efficacy in Sudden Deafness Patients with Comorbid Diabetes
【摘要】 目的 探究甲泼尼龙不同给药方式与突发性耳聋合并糖尿病患者疗效的相关性。方法 筛选合并糖尿病的首诊突发性耳聋患者作为研究对象,按甲泼尼龙的不同给药方式随机分三组:耳后注射组(例=41),鼓室注射组(例=45),静脉给药组(例=40),参照诊疗指南进行规范化治疗并统计治疗效果及空腹血糖水平。结果 (1)统计治疗有效率分别为耳后注射组78%,鼓室注射给药组:68.9%,静脉给药组:52.5%;(2)与静脉给药组相比,耳后注射与鼓室注射组高频听力明显改善(P<0.05);(3)静脉给药组治疗后血糖明显升高,且明显高于局部给药组治疗后的血糖水平(P<0.05);(4)按疗效重新分组,无论治疗是否有效,治疗后血糖均明显高于治疗前,且治疗无效组治疗后显著高于有效组(P<0.05)。结论 对首诊合并糖尿病的SSNHL患者,局部给药较静脉全身给药对血糖影响更小,疗效更好,值得在此类患者中推广运用。
【Abstract】 Objective To investigate the correlation between different administration methods of methylprednisolone and the therapeutic efficacy in patients with sudden deafness complicated by diabetes mellitus. Methods Patients with sudden sensorineural hearing loss(SSNHL) who were initially diagnosed and complicated by diabetes were selected as the study subjects. They were randomly divided into three groups according to different administration methods of methylprednisolone: retroauricular injection group(n=41), tympanic cavity injection group(n=45), and intravenous administration group(n=40). Standardized treatment was performed according to diagnostic and treatment guidelines, and the therapeutic effect and fasting blood glucose levels were recorded. Results(1)The statistical effective rates of treatment were 78% in the retroauricular injection group, 68.9% in the tympanic cavity injection group,and 52.5% in the intravenous administration group, respectively.(2) Compared with the intravenous administration group, the high-frequency hearing in the retroauricular injection and tympanic cavity injection groups was significantly improved(P<0.05).(3)After treatment, the blood glucose levels in the intravenous administration group increased significantly and were significantly higher than those in the local administration group(P<0.05).(4)When re-grouped according to therapeutic effect, regardless of whether the treatment was effective or not, the blood glucose levels after treatment were significantly higher than those before treatment, and the blood glucose levels in the ineffective treatment group were significantly higher than those in the effective treatment group(P<0.05). Conclusion For patients with SSNHL who are initially diagnosed and complicated by diabetes, local administration has a smaller impact on blood glucose levels and better therapeutic effect compared to intravenous systemic administration, making it worthy of promotion and application in such patients.
