基于血流动力学的腹主动脉瘤破裂风险的研究

【摘要】 目的 分析破裂与未破裂腹主动脉瘤(AAA)血流动力学的差异,探讨破裂AAA的风险因素。方法 收集2021年11月至2023年11月河北医科大学第一医院收治的39例AAA患者的临床资料,以1:2的比例分为破裂组(n=13)和未破裂组(n=26)。收集所有患者的计算机断层扫描血管成像(CTA)图像数据,并通过计算流体动力学(CFD)比较两组患者的血流动力学参数,分析AAA瘤体直径与血液流动速度、血管壁面压力之间的相关性。结果 破裂组患者的瘤体最大直径大于未破裂组患者,血管壁面压力和血液流动速度均小于未破裂组患者,差异均有统计学意义(P﹤0.05)。两组患者的WSS和瘤颈直径比较,差异均无统计学意义(P﹥0.05)。相关性分析结果显示,AAA的瘤体直径与血液流动速度呈负相关(r=-0.329,P=0.041),与血管壁面压力呈正相关(r=0.408,P=0.015)。结论 壁面压力、血液流动速度与AAA的破裂密切相关。

【Abstract】 Objective To analyze the differences in hemodynamics between ruptured and unruptured abdominal aortic aneurysms(AAA) and to explore the risk factors for AAA rupture. Method The clinical data of 39 AAA patients admitted to the First Hospital of Hebei Medical University from November 2021 to November 2023 were collected,and the patients were divided into ruptured group(n=13) and unruptured group(n=26) in a ratio of 1:2. The computed tomography angiography(CTA) image data of all patients were collected, and hemodynamic parameters of the two groups were compared through computational fluid dynamics(CFD), and the correlation between AAA tumor diameter,blood flow velocity, and vascular wall pressure were analyzed. Result The tumor maximum diameter in the ruptured group was larger than that in the unruptured group, the intraluminal pressure and blood flow velocity in the ruptured group were lower than those in the unruptured group, with statistically significant differences(P<0.05). There were no statistically significant differences in the WSS and aneurysm neck diameter between the two groups(P>0.05). The correlation analysis results showed that the tumor maximum diameter of AAA was negatively correlated with blood flow velocity(r=-0.329, P=0.041) and positively correlated with vascular wall pressure(r=0.408, P=0.015). Conclusion The wall pressure and blood flow velocity are closely related to AAA rupture.
