佛山市南海区女性宫颈HPV感染及基因型分布的研究
Prevalence and genotype distribution of cervical HPV infection among females in Nanhai District, Foshan City
【摘要】 目的 了解佛山市南海区女性宫颈人乳头瘤病毒(human papillomavirus,HPV)感染情况,阐明不同宫颈病变中HPV基因型的分布规律。方法 收集2020—2024年在南方医科大学第七附属医院体检或就诊的28 592例女性宫颈HPV分型结果,统计分析HPV亚型感染情况、年龄及临床诊断情况。结果 2020—2024年佛山市南海区女性HPV感染呈现显著的年龄“U”型分布(≤20岁和≥61岁组感染率高),且以高危型感染为主。高危型HPV 52、16、58是本地区宫颈病变的主要致病型别,其中HPV16感染率随病变严重程度显著升高。九价疫苗可覆盖本地区78.26%的宫颈癌相关HPV基因型,但预防总体HPV感染覆盖率为42.80%。结论 2020—2024年佛山市南海区女性HPV感染模式具有明显的年龄差异,五年感染率呈U型演变,建议加强适龄人群筛查与宣教、加强监测与疫苗推广,并持续监测非疫苗覆盖高危型别(如HPV51、68、39)的流行趋势。
【Abstract】 Objective To investigate the prevalence of human papillomavirus(HPV) infection among females in Nanhai District, Foshan City, and elucidate the distribution patterns of HPV genotypes across different cervical lesions. Methods HPV genotyping results from 28 592 female patients who underwent physical examinations or medical consultations at the Seventh Affiliated Hospital of Southern Medical University between 2020 and 2024 were collected. Statistical analyses were conducted on HPV subtype infection status, age distribution, and clinical diagnosis. Results From 2020 to 2024, HPV infection among females in Nanhai District, Foshan City, exhibited a distinct U-shaped age distribution(higher infection rates in the ≤20 and ≥61 age groups), with high-risk types predominating. High-risk HPV types 52, 16, and 58 were identified as the primary pathogenic variants in this region, with HPV16 infection rates showing a significant increase correlating with lesion severity. The 9-valent vaccine could cover 78.26% of cervical cancer-associated HPV genotypes in this region, but its overall HPV infection coverage rate was 42.80%. Conclusion The HPV infection pattern among females in Nanhai District,Foshan City from 2020 to 2024 demonstrated significant age-related disparities, with a U-shaped infection rate trend over the five-year period. It is recommended to enhance screening and education for age-eligible populations, strengthen surveillance and vaccine promotion, and continuously monitor the epidemiological trends of non-vaccine-covered high-risk types(e.g.,HPV 51, 68, 39).
