贵州省2011-2022年新报告青年学生HIV/AIDS患者特征及趋势分析
Characteristics and trends of newly reported HIV/AIDS cases among young students in Guizhou Province from 2011 to 2022
【摘要】 目的 了解2011-2022年贵州省新报告青年学生HIV/AIDS患者流行特征,为针对该人群制定防控策略提供科学依据。方法 通过“中国疾病预防控制信息系统艾滋病防治基本信息系统”,对2011-2022年贵州省新报告的全部15~24岁青年学生HIV/AIDS患者进行分析,包括地区、性别、年龄和时间分布,采用Joinpoint回归模型进行新发现率的趋势变化分析。结果 2011-2022年贵州省新报告15~24岁青年学生患者1 122例,年报告病例数从2011年的52例上升至2022年的107例,占当年15~24岁HIV/AIDS总数的比例呈总体上升趋势(χ趋势2=62.771,P<0.001),年新发现率从2011年的2.96/10万上升到2022年的4.45/10万。15~17岁组新发现率呈上升趋势(2011-2022年APC=9.03);18~22岁组呈先上升后下降的趋势(2011-2015年APC=9.59,2015-2022年APC=-5.73);23~24岁组新发现率先保持平稳后快速下降(2011-2017年APC=-1.11,2017-2022年APC=33.36);总体看2011-2017年青年学生HIV/AIDS患者新发现率呈上升趋势(APC=11.00),2017-2022年保持平稳(P≥0.05)。不同性别青年学生HIV/AIDS患者年龄、民族、受教育程度、病例发现途径、感染途径差异有统计学意义(P<0.001);通过医疗机构发现青年学生患者的比例呈逐年上升趋势(χ趋势2=15.706,P<0.001)。结论 2011-2022年,贵州省新报告15~24岁青年学生HIV/AIDS患者数总体呈现先上升后平稳的趋势,但15~17岁组新发现率呈上升趋势;男性以同性传播为主,女性以非婚非商业性行为为主,应将宣传教育关口前移,针对性地开展干预和检测工作。
【Abstract】 Objective To understand the characteristics of human immunodeficiency virus(HIV)/acquired immunodeficiency syndrome(AIDS) cases and sexual behaviors among young students in Guizhou Province and provide a scientific basis for formulating prevention and control strategies for this population. Methods All HIV/AIDS cases reported by young students aged 15-24 years in Guizhou Province were analyzed using the China Information System for AIDS Prevention and Control, including regional, gender, age, and time distribution. The joinpoint regression model was used to analyze the trend change in the new discovery rate. Results From 2011 to 2022, 1 122 new cases in young students aged 15-24 years were reported in Guizhou Province. The number of annually reported cases increased from 52 in 2011 to 107 in 2022, accounting for the total number of HIV/AIDS cases in those aged 15-24 years in that year. The proportion showed an overall upward trend(χtrend2=62.771, P=0.001). The annual new discovery rate was expected to increase from 2.96/100 000 in 2011 to 4.45/100 000 by 2022. The new discovery rate of the 15-17-age group showed an upward trend, with an annual percent change(APC) of 9.03% from 2011 to 2022. The 18-22-age group showed a trend of increasing first and then decreasing(2011-2015 APC=9.59%, 2015-2022 APC=-5.73%);in the 23-24-age group, the new findings first remained stable and then decreased rapidly(APC=-1.11% in 2011-2017, APC=33.36% in 2017-2022);overall, the new HIV/AIDS discovery rate of young students showed an upward trend from 2011 to 2017(APC=11.00%)and remained stable from 2017 to 2022(P<0.05). There were statistically significant differences in age, nationality, education level, case detection route, and infection route of HIV/AIDS among young students of different sexes(P< 0.001). The proportion of cases of young students found through medical institutions showed an increasing trend year by year(χtrend2=15.706, P=0.001). Conclusions In recent years, the number of HIV/AIDS cases reported by students aged 15-24 years in Guizhou Province has shown a relatively stable upward trend. The detection rate in the 15 – 17-age group showed an upward trend. Transmission among males occurred primarily through homosexual contact, whereas among females, it predominantly occurred via non-marital and non-commercial sexual behaviors. It is imperative to prioritize prevention education at earlier stages, implement targeted intervention strategies, and enhance testing measures for these populations.
