多模态超声成像技术在PRF矫正中、重度鼻唇沟皱纹术后效果中的应用价值

【摘要】 目的:通过联合高频超声测量皮肤真皮层厚度、CDFI和SMI评估皮肤血流灌注等多模态超声成像技术评估富血小板纤维蛋白(PRF)注射矫正鼻唇沟皱纹的术后效果。方法:36例鼻唇沟严重程度评定量表(WSRS)评分在3分或4分的受试者,应用PRF注射治疗,以高频超声采集其治疗前及治疗后各时间点的皮肤真皮层厚度、血流量,并将鼻唇沟注射前的超声数据作为参考基线,动态评估鼻唇沟治疗前后的真皮层厚度、血流量的效果差异。结果:与治疗前相比,WSRS评分治疗后1月和3月显著降低(2.53±0.56、2.61±0.55 vs 3.42±0.50,P均<0.01);全球审美改善量表GAIS评分治疗后1月和3月明显改善,治疗后6月则倾向无效;鼻唇沟皮肤真皮层厚度治疗后1月和3月显著增加(1.90±0.06、1.89±0.06 vs 1.59±0.08,P均<0.01)。临床评估的WSRS评分和超声定量测量皮肤厚度变化之间显著负相关关系(Rs=-0.729,P<0.01)。CDFI评估发现治疗后1-3月皮肤血流Adler分级显著增加(χ~2=59.204,P<0.01);SMI评估发现治疗后1-3月所有受试者皮肤血流信号增多丰富。结论:多模态超声成像技术凭借其对鼻唇沟皮肤的可视化,通过厚度、回声、血流评估PRF治疗后及远期效果,为鼻唇沟的年轻化治疗提供了一种新的测量评估方法。

【Abstract】 Objective:To evaluate the postoperative effect of platelet-rich fibrin(PRF)injection for correcting nasolabial fold wrinkles through multimodal ultrasound imaging techniques such as measuring the thickness of the dermal layer of the skin by combined high-frequency ultrasound,assessing skin blood perfusion by color Doppler flow imaging(CDFI)and superb microvascular imaging(SMI).Method:Thirty-six subjects with a nasolabial fold wrinkle severity rating scale(WSRS)score of 3or 4were treated with PRF injection.High-frequency ultrasound was used to collect the thickness of the dermal layer of the skin and blood flow at various time points before and after treatment.The ultrasound datas before nasolabial fold injection were taken as the reference baseline to dynamically evaluate the differences in the effects on the thickness of the dermal layer and blood flow before and after the treatment of the nasolabial fold.Results:Compared with before treatment,the WSRS score was significantly reduced after treatment 1month and 3months(2.53±0.56,2.61±0.55vs 3.42±0.50,all P<0.01).The global aesthetic improvement scale(GAIS)score showed significant improvement after treatment 1month and 3months,and tended to be ineffective 6months after treatment.The thickness of the dermal layer of the nasolabial fold skin was increased significantly after treatment 1month and 3months(1.90±0.06,1.89±0.06vs 1.59±0.08,all P<0.01).There was a significant negative correlation between the WSRS score in clinical assessment and the change in skin thickness measured quantitatively by ultrasound(Rs=-0.729,P<0.01).CDFI assessment found that the Adler grading of skin blood flow was significantly increased after treatment 1month and 3months(χ~2=59.204,P<0.01).SMI assessment found that the skin blood flow signals of all subjects were increased and became richer after treatment 1 month and 3months.Conclusion:Multimodal ultrasound imaging techniques,by virtue of their visualization of the nasolabial fold skin and through the evaluation of thickness,echo,and blood flow after PRF treatment and in the long term,provide a new measurement and evaluation method for the rejuvenation treatment of the nasolabial fold.
