学龄前儿童不同间隔时间序贯植入双侧人工耳蜗术后听觉效果分析
An Analysis of Auditory Effects in Preschool Children after Sequential Bilateral Cochlear Implantation at Different Intervals
【摘要】 目的 比较学龄前儿童不同间隔时间序贯植入双侧人工耳蜗术后听觉效果。方法 选取2022年2月~2023年5月在本院诊治的45例13~68个月学龄前儿童,按序贯植入的间隔时间分为双侧人工耳蜗序贯植入1组(sequential bilateral cochlear implantation I,SBCI1)10例(间隔<12个月)、SBCI2组23例(间隔12~23个月)及SBCI3组12例(间隔24~36个月),分别在首次人工耳蜗植入时(T0)、对侧耳蜗植入时(T1)及双侧耳蜗植入1年后(T2),采用父母版言语空间特性量表(the speech,spatial and qualities of hearing scale-parent’s version,SSQ-P)、儿童听力/口语评估家长问卷(parents’evaluation of aural/oral performance of children,PEACH)及言语可懂度分级问卷(SIR)评估患儿听觉康复效果。结果 (1)双侧人工耳蜗植入1年后(T2),3组儿童的听觉能力相近(P>0.05)。(2)双侧人工耳蜗植入1年后(T2),3组儿童的听觉能力较对侧人工耳蜗植入时(T1)显著改善;对侧耳蜗植入时(T1),3组儿童的听觉能力较首次植入时(T0)显著改善(P<0.01)。(3)T0~T1期间,SBCI2、SBCI3组的SSQ-P、PEACH得分增长水平高于SBCI1组;SBCI3组SIR得分增长水平显著高于SBCI1组(P<0.01)。T1~T2期间,SBCI1组的SSQ-P得分增长水平高于SBCI2,SBCI2组高于SBCI3组;SBCI1、SBCI2组PEACH得分增长水平显著高于SBCI3组(P<0.01)。结论 学龄前儿童不同间隔时间序贯植入双侧人工耳蜗术后短期听觉能力相近,但植入间隔时间越短的患儿获得听觉效果增益越早,应合理制订康复计划,促进患儿全面康复。
【Abstract】 Objective To compare the auditory effects of sequential implantation of bilateral cochlear implants at different intervals in preschool children.Methods 45 preschool children aged 13-68 months who were treated in our hospital from February2022 to May 2023 were selected and divided into SBCI1 group of 10 cases(interval<12 months),SBCI2 group of 23 cases(interval12-23 months),and SBCI3 group of 12 cases(interval 24-36 months) according to the interval of sequential implantation.At the time of the first cochlear implantation(T0),contralateral cochlear implantation(T1),and one year after bilateral cochlear implantation(T2),the auditory effects of the children were evaluated using The Speech,Spatial And Qualities Of Hearing Scale-Parent’s Version(SSQ-P)and Parents’ Evaluation Of Aural/Oral Performance Of Children(PEACH),Speech Intelligibility Rate(SIR).Results One year after bilateral cochlear implantation(T2),the auditory abilities of the three groups of children were similar(P>0.05).After one year of bilateral cochlear implantation(T2),the auditory ability of the three groups of children significantly improved compared to the contralateral cochlear implantation(T1).During contralateral cochlear implantation(T1),the auditory abilities of the three groups of children were significantly improved compared to the first cochlear implantation(T0)(P<0.01).From T0 to T1 the growth levels of SSQ-P and PEACH scores in the SBCI2 and SBCI3 groups were higher than those in the SBCI1 group.The SIR score growth level of SBCI3 group was higher than that of SBCI1 group(P<0.05).From T1 to T2,the SSQ-P score growth level in the SBCI1 group was higher than that in the SBCI2 group,and SBCI2 was higher than that in the SBCI3 group.The growth level of PEACH scores in SBCI1and SBCI2 groups was higher than that in SBCI3 group(P<0.01).Conclusion Preschool children who undergo sequential bilateral cochlear implantation at different intervals have similar short-term auditory abilities,but the shorter the implantation interval,the earlier the auditory effect gain.A precise rehabilitation plan should be developed to promote comprehensive recovery of the children.
