阿戈美拉汀对脑卒中后抑郁患者血清神经递质及睡眠质量的影响

【摘要】 目的 探讨阿戈美拉汀在脑卒中后抑郁患者中的应用效果。方法 选取2022年1月至2024年1月泉州市第三医院收治的96例脑卒中后抑郁患者,按随机数字表法分为两组。对照组(n=48)给予舍曲林治疗,观察组(n=48)给予阿戈美拉汀治疗。治疗8周,对比两组临床疗效、血清神经递质、睡眠质量、抑郁症状、神经功能及药物安全性。结果 观察组治疗总有效率(87.50%)高于对照组(68.75%),有统计学差异(P<0.05)。治疗后,观察组去甲肾上腺素[(35.73±4.52)ng/L]、5-羟色胺[(145.23±20.12)ng/L]、多巴胺[(65.24±12.35)pg/mL]及脑源性神经营养因子[(22.28±4.54)ng/mL]均高于对照组[(24.86±3.35)ng/L、(127.45±15.25)ng/L、(45.24±10.15)pg/mL、(18.45±3.25)ng/mL];观察组匹兹堡睡眠质量指数评分[(5.87±0.91)分]、汉密尔顿抑郁量表(24项)评分[(5.27±1.23)分]及美国国立卫生研究院卒中量表评分[(10.28±1.83)分]均低于对照组[(8.13±1.24)分、(7.23±1.37)分、(12.97±1.13)分],有统计学差异(P<0.001)。观察组不良反应发生率(8.33%)低于对照组(22.92%),有统计学差异(P<0.05)。结论 阿戈美拉汀治疗脑卒中后抑郁患者的效果显著,可更好地调节血清神经递质水平,改善患者睡眠质量、抑郁症状及神经功能,且不良反应较少。

【Abstract】 Objective To explore the effect of agomelatine in patients with post-stroke depression.Methods Using random numerical table method, 96 cases of post-stroke depression who treated in Quanzhou Third Hospital during the period of January 2022 to January 2024 were assigned to two groups. The control group(n=48) was treated with sertraline, while the observation group(n=48) was treated with agomelatine. After 8 weeks of treatment, the comparisons were performed on clinical efficacy, serum neurotransmitters, sleep quality, depressive symptoms, neurological function, and medication safety in both groups. Results The observation group obtained higher overall effective rate than the control group(87.50% vs 68.75%)(P<0.05). After treatment, the observation group showed higher levels of norepinephrine [(35.73±4.52) ng/L], 5-hydroxytryptamine [(145.23±20.12) ng/L], dopamine [(65.24±12.35) pg/mL], and brain-derived neurotrophic factor [(22.28±4.54) ng/mL] compared to the control group [(24.86±3.35) ng/L,(127.45±15.25) ng/L,(45.24±10.15) pg/mL,(18.45±3.25) ng/mL]. The observation group also had lower scores on the Pittsburgh Sleep Quality Index [(5.87±0.91) points], Hamilton Depression Scale(24-item) [(5.27±1.23) points], and National Institutes of Health Stroke Scale [(10.28±1.83) points] compared to the control group [(8.13±1.24) points,(7.23±1.37) points,(12.97±1.13) points](P<0.001). The observation group demonstrated lower incidence of adverse reactions compared with the control group(8.33% vs 22.92%)(P<0.05). Conclusion The therapeutic effect of agomelatine for patients with post-stroke depression is markedly effective, better regulating serum neurotransmitter levels, improving sleep quality, depressive symptoms and neurological function, with fewer adverse reactions.
