脓毒症相关低钙血症的发病机制及临床诊疗进展
Pathogenesis and advances in clinical diagnosis and treatment of sepsis-associated hypocalcemia
【摘要】 脓毒症作为一种因宿主对感染反应失调而引发的严重器官功能障碍,具有潜在的致命风险。血清钙作为一种关键矿物质,在细胞信号传递、肌肉收缩、凝血功能及免疫调节等生理过程中发挥重要作用。近年研究表明,脓毒症患者常出现钙稳态失衡现象,这种失衡可能与其临床预后存在显著关联。脓毒症相关低钙血症的发生机制涉及多个方面,包括细胞内Ca2+浓度异常升高、免疫系统功能紊乱以及内分泌激素代谢失调等。值得注意的是,脓毒症患者的血钙水平波动与甲状旁腺激素和维生素D的调节功能异常密切相关。尽管已有证据表明缺乏维生素D可能加剧脓毒症患者的病情进展,但其补充治疗的确切效果仍存在争议。目前,研究者正致力于探索钙通道阻滞剂及其他钙稳态调节治疗方案,有望为脓毒症的临床治疗开辟新的途径和策略。
【Abstract】 Sepsis, a severe organ dysfunction triggered by a dysregulated host response to infection, carries a potentially fatal risk. Serum calcium, a crucial mineral, plays a significant role in various physiological processes such as cellular signaling, muscle contraction, blood coagulation, and immune regulation. Recent studies have shown that calcium homeostasis imbalance is common in sepsis patients, and this imbalance may be significantly associated with their clinical prognosis. The mechanisms underlying sepsis-related hypocalcemia involve multiple aspects, including abnormal elevation of intracellular Ca2+ concentration, immune system dysfunction, and endocrine hormone metabolic disorders. Notably, fluctuations in blood calcium levels in sepsis patients are closely related to abnormal regulatory functions of parathyroid hormone and vitamin D. Although evidence suggests that vitamin D deficiency may exacerbate the progression of sepsis, the exact efficacy of its supplementation therapy remains controversial. Currently, researchers are focusing on exploring calcium channel blockers and other calcium homeostasis regulation treatment options, which are expected to open new avenues and strategies for the clinical treatment of sepsis.
