重视阿尔茨海默病和老年痴呆症临床与人群干预的转化研究
Strengthening translational research towards clinical and preventive interventions against dementia and Alzheimer’s disease
【摘要】 随着人口老龄化加剧,老年期痴呆已成为全球重大公共卫生问题。阿尔茨海默病(AD)是最常见的痴呆类型,约占所有临床痴呆病例的70%。尽管部分AD早期患者可使用改善病情的药物(例如利卡尼单抗和多纳尼单抗)治疗,但痴呆仍然无法治愈。然而,流行病学研究已识别出10多个可控的痴呆风险因素,如果针对这些因素采取干预措施,可以预防全球约45%的痴呆病例。事实上,同时针对多个可控风险因素以延缓痴呆发作的多模块干预措施已成为应对痴呆症巨大挑战的潜在替代策略。近年来,基于血液的AD(如p-tau217)、神经退行性疾病(如NfL)和神经炎症(如GFAP)生物标志物研究取得重要进展,从而能够更早和更准确地检测和诊断疾病,提供个性化治疗,并追踪病情进展和对药物治疗效果的反应。血液生物标志物提供了一种比基于传统脑脊液和PET扫描的生物标志物侵入性更低、更具成本效益的替代方案,但转化研究对于优化这些生物标志物的临床应用仍然至关重要。总之,我们呼吁采取行动加强转化研究,旨在将基础、临床和人群流行病学研究的科学发现有效地转化为AD和痴呆治疗性和预防性干预的实践。
【Abstract】 As population ages,dementia has become a global public health concern. Alzheimer’s disease(AD) is the most common type of dementia,accounting for 70% of all clinical dementia cases.Although disease-modifying medications are available for early-stage AD(lecanemab and donanemab) to slow cognitive and functional decline,there remains no cure for dementia.However,epidemiological research has identified over dozen modifiable risk factors for dementia,which,if targeted by active interventions,could prevent 45% of all dementia cases worldwide.Multidomain intervention approach that targets simultaneously several modifiable riskfactors hasemerged as a potential alternative strategy to deal with the huge challenges of dementia. In addition,recent research has developed blood-based reliable biomarkers for AD(e.g.,p-tau217),neurodegeneration(e.g.,NfL),and neuroinflammation(e.g.,GFAP),which allows for earlier accurate diagnosis of AD, personalized treatment, and tracking disease progression and responses to therapies. Bloodbasedbiomarkers offer a less invasive and more cost-effective alternative to traditional CSF and PETimaging biomarkers;however,translational research remains crucial for optimizing clinical application of these biomarkers.Taken together,we call for action to strengthen translational research that aims to efficiently translate scientific discoveries from basic, clinical, and epidemiological research into practice for therapeutic and preventive interventionsagainst dementia.
