钠葡萄糖协同转运蛋白2抑制剂对非糖尿病扩张型心肌病心力衰竭患者QTc间期的影响

【摘要】 目的 探讨钠葡萄糖协同转运蛋白2(SGLT2)抑制剂对非糖尿病扩张型心肌病患者心率校正的QT(QTc)间期的影响。方法 研究招募2019年1月至2022年3月在湘雅二医院住院的临床确诊的非糖尿病扩张型心肌病患者。根据是否使用SGLT2抑制剂分为SGLT2抑制剂组和对照组。收集两组患者基线时的人口统计学、生化指标、影像学检查和治疗方案数据,以及随访1年后的心电图和心脏超声结果。结果 治疗1年后,SGLT2抑制剂组与对照组相比,QTc间期[-29.5(-47.3,-12.0)比-12.5(-40.0,13.0),P=0.035]和左心室直径[-9.0(-16.0,-3.0)比-2.5(-10.0,1.0),P=0.017]均显著缩短。然而,QRS持续时间、其他心腔大小及左室射血分数方面没有发现明显差异。logistic回归分析显示,SGLT2抑制剂(HR 4.128,95%CI 1.285~13.266,P=0.017)是QTc间期缩短的独立预测因子。此外,相关性分析表明,QTc间期的变化与体重指数和白蛋白之间存在显著关联。结论 SGLT2抑制剂可改善非糖尿病扩张型心肌病患者的QTc间期。

【Abstract】 Objective to investigate the effect of Sodium-Dependent Glucose Transporter 2(SGLT2)inhibitor on QTc interval in patients with non-diabetic dilated cardiomyopathy. Methods The patients with clinically diagnosed nondiabetic dilated cardiomyopathy who were hospitalized in the Second Xiangya Hospital from January 2019 to March 2022were recruited for this study. They were categorized into SGLT2 inhibitor group and control group according to whether SGLT2 inhibitor was used or not. Demographics,biochemical indices,imaging tests and treatment regimen data at baseline,as well as electrocardiogram and cardiac ultrasound results after one year of follow-up,were collected from both groups.Results After one year of treatment,the QTc interval[-29.5(-47.3,-12.0)vs.-12.5(-40.0,13.0),P=0.035]and left ventricular diameter[-9.0(-16.0,-3.0)vs.-2.5(-10.0,1.0),P=0.017]in SGLT2 inhibitor group were both significantly shorterned compared to control group. However,no significant differences were found in QRS duration,other chamber sizes,or left ventricular ejection fraction.Logistic regression analysis showed that SGLT2 inhibitor(HR 4.128,95% CI 1.285-13.266,P=0.017)was an independent predictor of QTc interval shortening. In addition,correlation analysis showed a significant association between changes in QTc interval and body mass index and albumin. Conclusion SGLT2inhibitors may improve the QTc interval in patients with nondiabetic dilated cardiomyopathy.
