【摘要】 泌乳素增高是抗精神疾病类药物常见的不良反应之一，临床上泌乳素增高可以造成泌乳现象，或伴有月经不调甚至闭经。泌乳属于中医学“乳泣”的范畴，中医学认为乳汁的形成、分泌与冲脉相关，冲脉赖肾精荣养，肝、胃两经既连系冲脉，又相络乳房，故泌乳的病位在肝、脾胃、肾、冲脉。冲脉以气血阴阳“冲和”为常，而泌乳是“冲和”失常的典型表现。临床上，肝、脾胃、肾气机失调，热邪引动冲气是泌乳发生的关键。故治疗当以清降逆气为主，疏通乳络为辅，并着眼于回乳，使乳回而络通，方能治之无虞。同时，当考虑到药毒之弊，予以清热解毒之法。治疗全程慎用或禁用温燥药物，防止其引动逆气，加重病情。更多还原

【Abstract】 The prolactin increase is a common adverse reaction caused by anti-psychiatric drugs.Clinically, it will result in lactation, or be accompanied by irregular menstruation or even amenorrhea.Lactation falls into the category of “spontaneous lactation” in traditional Chinese medicine(TCM).In TCM,it is believed that the formation and secretion of milk are related to the thoroughfare vessel which depends on kidney essence and nourishment.Meanwhile, the two channels of the liver and stomach are connected to both the thoroughfare vessel and the breast, and thus the lactation disease is located in the liver, spleen and stomach, kidney, and thoroughfare vessel.The thoroughfare vessel is usually “thoroughfare harmony” of qi, blood, yin, and yang, and lactation is the typical manifestation of “thoroughfare harmony” disorder.Clinically, the liver, spleen, and stomach, kidney qi machinery disorders, and heat pathogen-induced qi are the key to the occurrence of this disease.Therefore, the treatment should be based on clearing down the reverse qi, with dredging the milk collateral as the supplement.Additionally, the focus should be on delactation to achieve delactation and dredge the collateral, thus treating the disease completely.At the same time, considering the adverse effect of drug toxicity, the method of clearing heat and removing toxicity should be provided.In the whole course of treatment, warm dryness drugs should be carefully used or prohibited to prevent them from triggering reverse qi and aggravating the condition.更多还原
