【摘要】 目的 分析使用超声筛查中孕期胎盘脐带入口位置异常的临床价值。方法 选取2021年9月至2023年7月聊城市人民医院收治的100例疑似中孕期胎盘脐带入口位置异常患者为研究对象，所有患者均进行二维彩色多普勒超声、三维彩色多普勒超声检查，将临床综合诊断结果作为金标准，比较二维彩色多普勒超声、三维彩色多普勒超声的诊断效能。结果 100例疑似胎盘脐带入口位置异常患者中临床综合诊断确诊阳性23例，阴性77例；三维彩色多普勒超声、二维彩色多普勒超声检查结果均检出阳性22例。维彩色多普勒超声检查的灵敏度为91.30%(21/23)、特异度为98.70%(76/77)、准确度为97.00%(97/100)，均高于二维彩色多普勒超声检查的78.26%(18/23)、94.81%(73/77)、91.00%(91/100)，差异均有统计学意义（χ~2=12.800、70.054、82.383,P均<0.05）。结论 采取三维彩色多普勒超声检查中孕期胎盘脐带入口位置异常患者，能够得到较为准确的诊断结果，诊断效能较高。更多还原

【Abstract】 Objective To analyze the clinical value of ultrasound screening for abnormal placental umbilical cord entrance position in middle pregnancy. Methods From September 2021 to July 2023, 100 patients with suspected abnormal placental umbilical cord entrance position in the second trimester of pregnancy admitted to Liaocheng People’s Hospital were selected as the research objects. All patients underwent two-dimensional color Doppler ultrasound and three-dimensional color Doppler ultrasound. The clinical comprehensive diagnosis results were regarded as the gold standard, and the diagnostic efficacy of two-dimensional color Doppler ultrasound and three-dimensional color Doppler ultrasound was compared. Results Among the 100 patients with suspected abnormal position of placental umbilical cord entrance, 23 cases were diagnosed as positive and 77 cases as negative by clinical comprehensive diagnosis. The results of three-dimensional color Doppler ultrasound and two-dimensional color Doppler ultrasound were positive in 22 cases. The sensitivity, specificity and accuracy of dimensional color Doppler ultrasonography were 91.30%(21/23), 98.70%(76/77) and 97.00%(97/100), which were higher than [78.26%(18/23), 94.81%(73/77), 91.00%(91/100)] of two-dimensional color Doppler ultrasonography, and the differences were statistically significant(χ~2=12.800, 70.054, 82.383, all P<0.05). Conclusion Three-dimensional color Doppler ultrasonography can obtain more accurate diagnostic results and higher diagnostic efficiency in patients with abnormal placental umbilical cord entrance position during middle pregnancy.更多还原
