基于“瘀毒”理论探讨中性粒细胞胞外诱捕网在肺癌中的研究进展
Research Progress on Neutrophil Extracellular Traps in Lung Cancer Based on the Theory of “Blood Stasis and Toxin”
【摘要】 中性粒细胞胞外诱捕网(NETs)是由活化的中性粒细胞释放的DNA-蛋白质复合网状结构,其通过介导炎症因子释放、促进肿瘤相关血栓形成及破坏微环境稳态,推动肺癌的病理进程。中医学认为,在正气亏虚基础上,瘀留于内,因瘀致毒,因毒生变,终致瘀毒互结,乃肺癌的核心病机之一。NETs介导的血液高凝状态及血流动力学改变可类比为“血脉瘀阻”之病机,其诱导炎症因子释放导致的微环境恶性失衡则对应“正气亏虚”背景下的“毒邪积聚”,两者互为因果,相互促进,共同形成“瘀毒互结”的病理状态。本文以肺癌为研究对象,将中医传统理论与现代医学相结合,阐述NETs与瘀、毒的相关性,对中医药通过抑制NETs形成发挥抗肺癌作用的机制进行归纳总结,以期为中医药治疗肺癌的科学内涵提供理论支持。
【Abstract】 Neutrophil extracellular traps(NETs) are DNA-protein complex reticular structures released by activated neutrophils. NETs promote the pathological progression of lung cancer by mediating the release of inflammatory factors,promoting tumor-associated thrombosis and disrupting the homeostasis of the microenvironment. Traditional Chinese medicine(TCM) believes that blood stasis accumulates internally and leads to toxin generation on the basis of a deficiency of healthy qi,which subsequently induces pathological changes,and ultimately results in the interweaving of stasis and toxins,which is one of the core pathogenic mechanisms of lung cancer. The hypercoagulable state of blood and hemodynamic changes mediated by NETs can be analogized to the TCM pathogenesis of “ obstruction of blood vessels due to blood stasis ”,while the NETs-induced release of inflammatory factors leading to malignant imbalance of the microenvironment corresponds to the “accumulation of pathogenic toxins” arising from “deficiency of healthy qi”. These two processes are causally linked and mutually reinforcing,jointly forming the pathological state of “intermingled stasis and toxin ”. Focusing on lung cancer,this article integrates traditional Chinese theory with modern medical research,elaborates on the correlation between NETs and the TCM concepts of stasis and toxin,and summarizes the mechanisms by which Chinese medicine exerts anti-lung cancer effects through the inhibition of NETs formation,with a prospect of providing scientific support for the therapeutic rationale of Chinese medicine in lung cancer treatment.
