不同血清25-羟基维生素D水平早产儿感染情况、使用抗菌药物情况及住院天数比较

【摘要】 目的 比较不同血清25-羟基维生素D[25(OH)D]水平早产儿感染情况、使用抗菌药物情况及住院天数,为优化早产儿维生素D补充策略、降低感染发生率、改善早产儿生存质量提供科学依据。方法 选取2019年7月—2023年12月广西壮族自治区人民医院收治的早产儿136例,根据血清25(OH)D水平不同分为低维生素D组[25(OH)D≤50 nmol/L,65例]和正常维生素D组[25(OH)D>50 nmol/L,71例],比较两组感染情况、使用抗菌药物情况及住院天数。结果 低维生素D组发生早期感染49例(75.4%)、发生晚期感染21例(32.3%),正常维生素D组分别为39例(54.9%)、15例(21.1%),两组发生早期感染比例比较,P<0.05。低维生素D组早期使用抗菌药物(9.0±9.8)种/日、晚期使用抗菌药物(3.3±5.7)种/日,正常维生素D组分别为(5.6±6.8)、(2.7±6.7)种/日,两组早期使用抗菌药物情况比较,P<0.05。低维生素D组、正常维生素D组住院天数分别为(28.8±11.3)、(23.7±10.5)天,两组比较,P<0.05。结论 相较于正常血清25(OH)D水平早产儿,低血清25(OH)D水平早产儿更容易发生早期感染,且早期使用抗菌药物种类多和时间长,住院天数长。

【Abstract】 Objective To compare the infection status, antibiotic usage and hospital stay of premature infants with different serum 25-hydroxyvitamin D [25(OH)D] levels, in order to provide scientific basis for optimizing the vitamin D supplementation strategy for premature infants, reducing the incidence of infection and improving the quality of life of premature infants. Methods A total of 136 premature infants admitted to the People’s Hospital of Guangxi Zhuang Autonomous Region from July 2019 to December 2023 were selected. They were divided into the low vitamin D group [25(OH)D ≤ 50 nmol/L, 65 cases] and the normal vitamin D group [25(OH)D > 50 nmol/L, 71 cases] based on the serum 25(OH)D levels. The infection status, use of antibiotics, and hospital stay of the two groups were compared. Results In the low vitamin D group, 49 cases(75. 4%) had early infections and 21 cases(32. 3%) had late infections, and in the normal vitamin D group, the numbers were 39 cases(54. 9%) and 15 cases(21. 1%), respectively; there was a significant difference in the proportion of early infection between the two groups(P<0. 05). In the low vitamin D group, the early use of antibiotics was(9. 0 ± 9. 8) types per day and the late use was(3. 3 ± 5. 7) types per day, and in the normal vitamin D group, they were(5. 6 ± 6. 8) and(2. 7 ± 6. 7) types per day, respectively; significant difference was found in the early antibiotic use between the two groups(P<0. 05). The length of hospital stay was(28. 8 ± 11. 3) days in the low vitamin D group and(23. 7 ± 10. 5) days in the normal vitamin D group, with significant difference between the two groups(P<0. 05). Conclusion Compared with premature infants with normal serum 25(OH)D levels, those with low serum 25(OH)D levels are more likely to develop early infections, and they require more types of antibiotics used earlier, a longer duration of antibiotic use, and a longer hospital stay.
