成人前臂后皮神经感觉电位正常值的研究及其在臂丛神经损伤定位诊断中的价值

【摘要】 目的 测定正常成人前臂后皮神经感觉电位的正常值,并初步探索其在臂丛神经损伤定位诊断中的价值。方法 采用逆向法检测正常成人前臂后皮神经的感觉神经动作电位(sensory nerve action potentials, SNAP),估算正常成人前臂后皮神经SNAP的波幅负峰值及感觉传导速度(sensory conduction velocity, SCV)的正常值。并观察不同部位臂丛神经损伤患者前臂后皮神经的受损情况。结果 共收集正常成人前臂后皮神经电生理数据176条,其中男性74条,女性102条。统计分析显示波幅及SCV参考范围低限值分别为5.58μV及50.51 m/s。在16例有临床指征的臂丛神经损伤患者中,前臂后皮神经神经异常10例,正常6例。结论 前臂后皮神经可作为臂丛神经损伤患者肌电图的常规性检测项目,其在臂丛神经损伤的定位诊断中具有重要作用。

【Abstract】 Objective To measure the normal value of sensory nerve action potential of posterior antebrachial cutaneous nerves of normal adults and explore its value in the localization diagnosis of brachial plexus injury. Methods A reverse method was used to detect sensory nerve action potential(SNAP) of posterior antebrachial cutaneous nerves of normal adults. Normal values of negative peak amplitude of SNAPs and sensory conduction velocity(SCV) were estimated. Posterior antebrachial cutaneous nerve damages in patients with brachial plexus injuries at different locations were observed. Results A total of 176 electrophysiological data items of posterior antebrachial cutaneous nerves were collected from normal adults, including 74 males and 102 females. Statistical analysis showed that the low limits of wave amplitude and SCV reference range were 5.58 μV and 50.51 m/s, respectively. Among 16 patients with clinical indications of brachial plexus injury, 10 patients had abnormalities in posterior antebrachial cutaneous nerves, and 6 patients were normal. Conclusion Posterior antebrachial cutaneous nerve can be used as a routine detection item for electromyography in patients with brachial plexus injury, and it plays an important role in the localization diagnosis of brachial plexus injury.
