【摘要】 目的 观察急性混合型下肢深静脉血栓患者接受经胫后静脉进行导管接触性溶栓(CDT)联合机械性吸栓(PMT)治疗的效果。方法 回顾性分析76例急性混合型下肢深静脉血栓患者资料，采用倾向性匹配模型，将患者分为对照组(单纯胫后静脉行CDT治疗)和观察组(经胫后静脉行CDT联合PMT治疗),每组38例。分析或观察静脉通畅情况、下肢肿胀缓解程度及手术并发症。结果 2组患者治疗前静脉通畅情况、患健侧腿周径比较差异无统计学意义(P>0.05)。2组患者治疗效果比较差异有统计学意义(P<0.05)。结论 对急性混合型下肢深静脉血栓患者实施经胫后静脉行CDT联合PMT治疗，能显著改善患者的下肢深静脉血栓程度，提高静脉通畅率，其安全性较高、并发症少，可于临床中推广应用。更多还原

【Abstract】 Objective To observe the effect and value of combined catheter-directed thrombolysis(CDT) and mechanical thrombectomy(PMT) in the treatment of acute mixed lower extremity deep vein thrombosis.Methods A retrospective analysis was conducted on 76 patients with acute mixed lower extremity deep vein thrombosis treated.A propensity score matching model was used to divide the patients into a control group(treated with CDT via the posterior tibial vein only) and an observation group(treated with CDT combined with PMT via the posterior tibial vein),with 38 cases in each group.Analyze or observe venous patency, the degree of relief of lower limb swelling, and surgical complications.Results There was no statistically significant difference in venous patency and circumference comparison between the affected and healthy legs before treatment in the two groups of patients(P>0.05).There was a statistically significant difference in treatment efficacy between the two groups(P<0.05).Conclusion For patients with acute mixed-type deep vein thrombosis(DVT) of the lower limbs, the implementation of catheter-directed thrombolysis(CDT) combined with percutaneous mechanical thrombectomy(PMT) through the posterior tibial vein can improve the severity of deep vein thrombosis in the lower limb, and enhance venous patency rates, and demonstrate high safety with few complications, making it suitable for clinical application and promotion.更多还原
