【摘要】 目的 探讨多参数流式细胞术(MP-FCM)检测多发性骨髓瘤(MM)免疫表型及其意义。方法 抽取MM患者95例，均采用MP-FCM检测，分析患者MM的免疫表型。结果 95例MM患者中，Durie-Salmon临床分期为Ⅰ期的有10例，为Ⅱ期的有15例，为Ⅲ期的有70例。其中白细胞分化抗原(CD)38、CD138阳性表达率均为100.0%,CD56阳性表达率为65.3%,Cκ阳性表达率为50.5%,CD117阳性表达率为41.1%,CD28与HLA-DR、CD13阳性表达率分别为27.4%、25.3%、22.1%,其余指标阳性表达率均在10%以下；CD45在不同性别、不同Durie-Salmon分期的表达比较差异无统计学意义(P>0.05);CD33阳性表达男性为(67.58±18.00)%,女性为(41.33±14.32)%,性别比较差异有统计学意义(P<0.05);CD117阳性表达Ⅰ+Ⅱ期为(78.01±11.85)%,低于Ⅲ期的(37.44±13.79)%,Cλ阳性表达Ⅰ+Ⅱ期为(95.94±2.91)%,Ⅲ期为(90.32±6.63)%,各期比较差异有统计学意义(P<0.05),其余免疫指标在不同Durie-Salmon分期中表达比较差异无统计学意义(P>0.05);CD117、Cλ表达与Durie-Salmon临床分期具有一定关联性(P<0.05)。结论 应用MP-FCM同时检测多个CD的表达可作为MM诊断和疗效监测的重要手段，值得临床推荐。更多还原

【Abstract】 Objective To explore the immunophenotype and significance of multi parameter flow cytometry(MP-FCM) in detecting multiple myeloma(MM).Methods 95 MM patients were selected,the immunephenotype of MM patients were tested by MP-FCM.Results In 95 patients with MM,Durie Salmon clinical staging showed 10 cases in stage Ⅰ,15 cases in stage Ⅱ,and 70 cases in stage Ⅲ.The positive expression rates of CD38 and CD138 were both 100.0 %,followed by 65.3% for CD56,50.5% for Cκ,41.1 % for CD117,27.4% for CD28,25.3% for HLA-DR,and 22.1 % for CD13.The positive expression rates of other indicators were all below 10%;There was no statistically significant difference in the expression of CD45 among different genders and Durie Salmon stages(P>0.05);The positive expression of CD33 in males was 39.24%±17.24%,it was lower than that in females(41.33%±14.32%),showed statistically significant difference between genders(P<0.05);CD117 positive expression in stage Ⅰ+Ⅱ was 78.01 %±11.85 %,it was lower than stage Ⅲ(37.44 %±13.79 %),cLambda positive expression in stage Ⅰ+Ⅱ was 95.9%±2.91%,it was higher than stage Ⅲ(90.32%±6.63%)(P<0.05).Other immune indicators showed no difference in different Durie Salmon staging tables(P<0.05);There was a certain correlation between the expression of CD117 and Cλ and the clinical staging of Durie Salmon(P<0.05).Conclusion MP-FCM detecting the expression of multiple human leukocyte differentiation antigens can serve as an important approach for the diagnosis and efficacy monitoring of MM,and is worthy of clinical recommendation.更多还原
