微汗法论治尿毒症皮肤瘙痒症经验
Experience in Treating Uremic Pruritus with the Slight Sweating Therapy
【摘要】 尿毒症皮肤瘙痒症作为慢性肾衰竭最常见的并发症之一,严重威胁着患者的生命安全,也极大降低了患者的生活生存质量。由于其发病机制尚不明确,目前无确切有效的治疗手段。中医通过辨证论治,为尿毒症皮肤瘙痒症的治疗开辟了新的途径和思考的方向。文章总结了李琦教授治疗此病的临证经验,李琦教授指出本病的基本病机为脾肾阳虚,在此基础上,或表寒入里,或阳虚寒凝,均可致肌肤失养而引发瘙痒,临床上运用微汗法以解表祛邪或温化寒饮,收到满意效果。
【Abstract】 Uremic pruritus(UP),as one of the most common complications of chronic renal failure, poses a severe threat to patients’ lives and significantly reduces their quality of life. Due to its unclear pathogenesis, there is currently no definitive and effective treatment. Traditional Chinese Medicine(TCM) has opened up new avenues and directions for thinking in the treatment of UP through syndrome differentiation and treatment. This article summarizes Professor Li Qi’s clinical experience in treating this disease. Professor Li Qi points out that the basic pathogenesis of this disease is spleen-kidney yang deficiency. On this basis, either exterior cold invading the interior or yang deficiency leading to cold coagulation can cause the skin to be deprived of nourishment and trigger itchiness. Clinically, the mild sweating method is used to release the exterior and eliminate pathogens or to warm and resolve cold fluids, achieving satisfactory results.
