听力正常的美国成人耳鸣与全因和特因死亡率的关系
Association of tinnitus with all-cause and cause-specific mortality in in US adults with normal hearing
【摘要】 目的 研究美国20~69岁听力正常的成年人中,耳鸣与全因和病因特异性死亡率的关系。方法 这项以人群为基础的横断面研究使用了关于耳鸣参数和其他协变量的数据,如20~69岁参与者的年龄,身高和社会人口统计学特征。从耳鸣问卷日期到死亡日期或2019年12月31日进行评估。通过问卷调查来评估耳鸣的存在和耳鸣的严重程度。主要结果是受访者是否在访谈过程中死亡。采用倾向评分匹配和根据所有因素进行调整的cox回归模型来分析耳鸣和死亡率之间的关系。同时还进行了亚组分析。结果 与非耳鸣组(n=5 334)相比,耳鸣组(n=6 258)的平均年龄更高(3.32±0.49)岁vs (38.52±0.34)岁。耳鸣受试者因各种原因[风险比(Hazard Ratio, HR),1.54;95%CI(Confidence Interval,CI),1.06~2.24]和癌症(HR,3.62;95%CI:1.93~6.78)导致的死亡风险更高。耳鸣与意外死亡风险(HR,1.66;95%CI:0.61~4.52)和其他原因导致的死亡(HR,1.01;95%CI:0.47~2.16)的增加相关。相比之下,耳鸣并不影响心脏病的死亡率(HR,1.03;95%CI:0.40~2.62)。结论 与无耳鸣的患者相比,有耳鸣的患者因全因、癌症和意外死亡的死亡风险更高,但耳鸣并不影响心脏病的死亡率。这些发现还需要进一步的研究来证实。
【Abstract】 Objective To examine the correlation between tinnitus and mortality rates, both overall and specific to certain causes, in adults aged 20-69 years who have normal hearing in the United States.Methods This population-based cross-sectional study utilized data from five cycles of the National Health and Nutrition Examination Survey(NHANES) to examine the parameters of tinnitus and various covariates, including age, height, and sociodemographic characteristics, among participants aged 20-69 years.Dates were evaluated from the tinnitus questionData were analyzed based on the tinnitus-related questionsnaire date to the death date or December 31,2019.The presence of tinnitus and the severity of tinnitus were assessed with questionnaire Dates were evaluated from the tinnitus questionnaire date to the death date or December 31,2019.The presence of tinnitus and the severity of tinnitus were assessed with questionnaires.The primary outcome was to assess the impact of noise exposure on the respondent’s health, with a focus on mortality during the interview.Propensity score matching and cox regression models adjusted for all factors were used to analyze the association between tinnitus and mortality.Subgroup analyses were also conducted.Results Compared with the non-tinnitus group(n=5334),the tinnitus group(n=6258) had a higher mean(SD) age(33.32(0.49) years vs 38.52(0.34) years).Subjects with tinnitus had a higher risk of death from all causes(HR,1.54; 95%CI,1.06~2.24) and from cancer(HR,3.62; 95%CI,1.93~6.78).Tinnitus was associated with an increased risk of accidental death(HR,1.66; 95%CI,0.61~4.52) and death from other causes(HR,1.01; 95%CI,0.47~2.16).In contrast, tinnitus did not affect the mortality of cardiac disease(HR,1.03; 95%CI,0.40~2.62).Conclusion Subjects with tinnitus had a higher risk of mortality from all-cause, cancer, and accidental death compared to those without tinnitus, but tinnitus did not affect the mortality of cardiac disease.These findings require further studies to confirm.
