【摘要】 目的 探讨上消化道穿孔急诊手术患者术后肺部感染的危险因素。方法 以江门市中心医院2015年10月至2023年10月确诊上消化道穿孔并行急诊手术的350例患者为研究对象，根据是否出现术后肺部感染分成感染组（280例）和非感染组（70例），应用单因素和多因素Logistic回归分析分析肺部感染的危险因素。结果 单因素分析显示，感染组吸烟（P <0.001）、高血压（P=0.008）、术前胃管植入（P<0.001）、休克（P <0.001）、糖尿病（P <0.001）、穿孔原因（P=0.004）的占比均高于非感染组。多因素Logistic回归分析结果显示，吸烟、高血压是上消化穿孔术后肺部感染的独立危险因素（P <0.05）。结论 吸烟和高血压是上消化道穿孔急诊手术术后发生肺部感染的危险因素。更多还原

【Abstract】 Objective To investigate the risk factors for pulmonary infection following emergency surgery for upper gastrointestinal perforation. Method A total of 350 patients diagnosed with upper gastrointestinal perforation who underwent emergency surgery at our hospital were analyzed. Based on the presence of postoperative pulmonary infection, the patients were divided into an infection group and a non-infection group. Univariate and multivariate logistic regression analyses were conducted to identify the risk factors for pulmonary infection. Result Univariate analysis revealed that the infection group had a higher proportion of patients with a history of smoking(P < 0.001), hypertension(P=0.008), preoperative gastric tube placement(P < 0.001), shock(P < 0.001), diabetes(P < 0.001), and reason for perforation(P=0.004)compared to the non-infection group. Multivariate logistic regression analysis indicated that a history of smoking and hypertension are independent risk factors for pulmonary infection following surgery for upper gastrointestinal perforation(P < 0.05). Conclusion A history of smoking and hypertension are significant risk factors for the occurrence of pulmonary infection after emergency surgery for upper gastrointestinal perforation.更多还原
