【摘要】 目的：探讨儿童尿布区域发生白癜风的可能原因。方法：回顾性分析2024年4月至2024年9月在武汉市第一医院皮肤科就诊的4例尿布区域白癜风患儿资料，包括一般情况、临床表现、辅助检查、治疗和随访结果。结果：4例患儿的白斑部位均局限于会阴、外阴、肛周及腹股沟等尿布区域，白斑处无痛痒等自觉症状，边界不清，伍德灯检查、皮肤镜及反射式共聚焦显微镜（RCM）检查结果均符合白癜风表现，临床均诊断为白癜风。患儿停用一次性纸尿裤并接受常规白癜风治疗，3个月后患儿皮损较前好转。结论：儿童尿布区域白癜风的产生，可能与一次性纸尿裤的接触以及所致的炎症反应进一步诱发同形反应有关，这对儿童白癜风的防治有一定的临床意义。更多还原

【Abstract】 Objective: To discuss the possible causes of vitiligo in diaper area of children. Methods: The data of four children treated for vitiligo in the Dermatology Department of Wuhan First Hospital from April to September, 2024 were collected,including the general characteristics, vitiligo lesions, examinations, treatments and follow-up information. Results: In all the four cases, the leukoplakia was confined in an area covered by diapers(perineum, vulva, crissum and inguinal region), and there were no symptoms such as itch and pain at the lesion site, and the boundary was unclear. The results of Wood’s lamp, dermatoscopy and microscopy supported the diagnosis of vitiligo. Afterward, disposable diapers were not used any longer,and routine vitiligo treatment was initiated. Three months later, the vitiligo lesions showed benign changes. Conclusion: The occurrence of vitiligo in the child’s diaper area may be attributed to the contact with disposable diapers and the resulting inflammatory response, which in turn may trigger the Koebner phenomenon. Vigilance should be paid to the possibility of vitiligo in children using diapers.更多还原
