【摘要】 报告1例胫前局限毛发红糠疹。患者男，33岁。因左小腿胫前红斑、鳞屑及丘疹1年就诊。皮肤科检查：左小腿胫前手掌大红斑及毛囊性丘疹，皮损局限，表面粗糙，上覆弥漫性黄白色鳞屑，部分融合成片。皮损组织病理检查：毛囊扩张，内含毛囊角栓，棘细胞间轻度水肿，真皮浅层血管扩张，管周以淋巴细胞为主的炎性细胞浸润，表皮角质层的水平方向及垂直方向上可见交替出现的角化不全及角化亢进。诊断：毛发红糠疹。治疗：予维A酸乳膏、卤米松软膏外用，清热凉血润燥中药汤剂冷湿敷治疗，治疗14 d后皮损大部分消退。更多还原

【Abstract】 A case of tibial anterior localized pityriasis rubra pilaris is reported. A 33-year-old male presented with localized erythema, papules, and scales on the shin of the left lower leg for 1 year. Dermatological examinations showed an area of erythema with the size of a palm and follicular papules on the anterior tibia of the left lower leg. The skin lesions were localized, with a rough surface, covered with diffuse yellowish-white scales, some of which were fused into patches. Histopathological examination of the lesion revealed mild spongiosis with alternating hyperkeratosis and parakeratosis in horizontal and vertical directions. The hair follicles were dilated and filled with keratotic plugs. There were dilated blood vessels with a perivascular inflammatory cell infiltrate composed of predominantly lymphocytes in the superficial dermis. A diagnosis of pityriasis rubra pilaris was made. The patient was treated with topical application of tretinoin cream and halometasone ointment, and cold wet compress with traditional Chinese medicine decoction for clearing heat, cooling blood, and moistening dryness. After treatment, most of the skin lesions were resolved with good curative effect.更多还原
