【摘要】 目的 探讨全髋关节置换治疗老年股骨颈骨折的疗效。方法 采用全髋关节置换治疗58例老年股骨颈骨折患者。记录手术时间、术中出血量、疼痛VAS评分及并发症发生情况。采用髋关节功能Harris评分评定疗效。结果 手术时间61～135(96.88±17.07) min，术中出血量100～300(206.14±46.36) ml，术后第2天疼痛VAS评分1～3(1.97±0.77)分。患者均获得随访，时间12～20个月。术后1例输血，1例出现切口愈合不良，2例出现下肢肌间静脉血栓，无血管神经损伤、髋关节脱位及假体松动、断裂、周围骨折等并发症发生。术后1年髋关节功能Harris评分为79～98(89.21±5.23)分，其中优33例，良23例，可2例，优良率为96.55%;膝关节屈曲90°～125°(105.46°±10.57°)，伸直0°。结论 全髋关节置换治疗老年股骨颈骨折关节功能恢复较好，但需做好手术适应证的选择。更多还原

【Abstract】 Objective To explore the efficacy of total hip arthroplasty in the treatment of femoral neck fracture in the elderly. Methods Total hip arthroplasty was used to treat 58 elderly patients with femoral neck fractures. The operation time,intraoperative blood loss,pain VAS and the occurrence of complications were recorded. The therapeutic effect was evaluated by the Harris score of hip joint function. Results The operation time was 61 ～ 135( 96. 88 ±17. 07) min,the intraoperative blood loss was 100 ～ 300( 206. 14 ± 46. 36) ml,and the pain VAS was 1 ～ 3( 1. 97 ±0. 77) points on the second day after the operation. All patients were followed up for 12 ～ 20 months. After the operation,one case received blood transfusion,one case had poor incision healing,and two cases had intermuscular venous thrombosis of the lower extremities. No complications such as vascular and nerve injuries,hip dislocation,or loosening and fracture of the prothesis,periprosthetic fracture occurred. One year after the operation,the Harris score of hip joint function was 79 ～ 98( 89. 21 ± 5. 23) points. Among them,33 cases were excellent,23 cases were good,and 2 cases fair,the excellent and good rate was 96. 55%. The knee flexion was 90° ～ 125°( 105. 46° ± 10. 57°),and the extension was 0°． Conclusions For the treatment of femoral neck fractures in the elderly,total hip arthroplasty has a relatively good recovery of joint function,but it need to careful select the surgical indication.更多还原
