基于电感耦合等离子体质谱技术建立江西地区成人全血十种必需微量元素的参考区间

【摘要】 目的 基于电感耦合等离子体质谱(ICP-MS)技术建立成人全血10种必需微量元素的参考区间,充分考虑性别和年龄因素,以提供准确的微量元素水平评估依据,支持健康监测和疾病诊断。方法 纳入2023年6—7月健康体检的2 676名成年参考个体,根据性别和年龄分组,采用ICP-MS技术检测全血标本10种必需微量元素浓度,并采用非参数法确定参考区间。验证阶段纳入847名参考个体,计算验证数据超出参考区间比例。结果 所有参考区间建立和验证参考个体数均满足相关要求,验证数据超限比例均<10%。经统计分析,钒、钴、硒、锡不需要区分亚组,参考区间分别为0~1.89、0~0.51、96.18~204.66、0~5.18μg/L;锰、锶、铜、锌需要按照性别设置参考区间,男性分别为4.67~14.87μg/L、9.76~30.01μg/L、0.58~0.96 mg/L、4.50~7.68 mg/L,女性分别为5.05~17.78μg/L、10.19~32.19μg/L、0.63~1.04 mg/L、4.10~6.85 mg/L;钼需要按照年龄设置参考区间,20~39岁为0~1.16μg/L,40~49岁为0~1.40μg/L,50~79岁为0~1.72μg/L;铁则需要综合按照性别和年龄划分参考区间,具体为367.83~528.44 mg/L(男,20~59岁)、318.96~519.02 mg/L(男,60~79岁)、299.09~479.91 mg/L(女,20~79岁)。结论本研究成功建立了成人全血10种必需微量元素的参考区间,并验证了其适用性,为人体微量元素水平的准确评估提供了重要依据。但不同实验室差异可能存在,建议在引用这些参考区间时进行验证并考虑可能的差异,必要时建立自己实验室的参考区间。

【Abstract】 Objective To establish reference intervals for ten essential trace elements in adult whole blood using inductively coupled plasma mass spectrometry(ICP-MS) technology, taking into account gender and age factors. These reference intervals are essential for accurate assessment of trace element levels, supporting health monitoring and disease diagnosis. Methods A total of2 676 healthy adult reference individuals who underwent health examinations from June to July 2023 were included. They were grouped by gender and age, and the concentrations of ten essential trace elements in whole blood were measured using ICP-MS.Reference intervals were determined using non-parametric methods. In the validation phase, 847 reference individuals were included, and the proportion of validation data exceeding the reference intervals was calculated. Results All requirements for the establishment and validation of reference intervals were met, with a proportion of validation data exceeding the limits of <10%.Based on statistical analysis, vanadium, cobalt, selenium, and tin did not require differentiation by subgroups, with reference intervals of 0~1.89, 0~0.51, 96.18~204.66, and 0~5.18 μg/L, respectively. Manganese, strontium, copper, and zinc required genderspecific reference intervals, with ranges of 4.67~14.87 μg/L, 9.76~30.01 μg/L, 0.58~0.96 mg/L, and 4.50~7.68 mg/L for males and5.05~17.78 μg/L, 10.19~32.19 μg/L, 0.63~1.04 mg/L, and 4.10~6.85 mg/L for females, respectively. Molybdenum required agespecific reference intervals: 0~1.16 μg/L for ages 20~39, 0~1.40 μg/L for ages 40~49, and 0~1.72 μg/L for ages 50~79. Iron required composite reference intervals based on gender and age, specifically 367.83~528.44 mg/L(males, ages 20 ~59), 318.96 ~519.02 mg/L(males, ages 60~79), and 299.09~479.91 mg/L(females, ages 20~79). Conclusion This study successfully established reference intervals for ten essential trace elements in adult whole blood and validated their applicability, providing a crucial basis for the accurate assessment of trace element levels in the human body. However, inter-laboratory differences may exist, and it is recommended to validate and consider potential variations when referring to these reference intervals. If necessary, establish reference intervals specific to your laboratory.
