苏州市3~17岁儿童青少年去脂体重分布及百分位数曲线构建

【摘要】 目的 描述苏州市3~17岁儿童青少年去脂体重(FFM)和去脂体重指数(FFMI)分布特征,建立百分位数曲线。方法 2023—2024年采用多阶段分层整群随机抽样方法,以苏州市4个区县8 513名3~17岁儿童青少年为调查对象,采用问卷调查收集基本信息,测量身高、体重,利用生物电阻抗仪测量FFM,计算体质指数(BMI)和FFMI。采用GAMLSS法构建性别、年龄别FFM和FFMI百分位参考曲线。结果 男、女生FFM(男:Z=74.74,女:Z=67.55)、FFMI(男:Z=56.13,女:Z=40.03)中位数均随年龄增加而上升(P值均<0.05)。男生FFM(Z=-23.33~-2.32)和FFMI(Z=-21.24~-3.46)中位数均高于同龄女生(P值均<0.05),且在11岁后差距迅速增大。苏州市3~17岁儿童青少年FFM和FFMI生长曲线与华东地区的增长趋势基本一致,苏州市5~17岁男生(Z=-13.47~-2.71)和4~17岁女生(Z=-9.48~-2.82)的FFM中位数、5岁及7~17岁男生(Z=-8.76~-5.00)和3~7岁、11岁、13~15岁及17岁女生(Z=-4.90~-2.15)的FFMI中位数均高于华东地区(P值均<0.05)。结论 不同营养状态、不同性别儿童青少年FFM和FFMI增长存在一定差异,11岁后男、女生FFM和FFMI差异迅速增加。苏州市3~17岁儿童青少年FFM和FFMI生长曲线与华东地区增长趋势基本一致,且中位数普遍高于华东地区。

【Abstract】 Objective To describe the distribution characteristics of fat free mass(FFM) and fat free mass index(FFMI) in children and adolescents aged 3-17 in Suzhou, and to establish percentile curves. Methods A multi-stage stratified cluster random sampling method was used to survey 8 513 children and adolescents aged 3-17 in four districts and counties of Suzhou from 2023 to 2024.A questionnaire survey was used to collect their basic information, and their heights and weights were measured.A bioelectrical impedance meter was used to measure FFM,and to calculate body mass index(BMI) and FFMI.GAMLSS method was used to construct percentile reference curves for gender-based and age-based FFM and FFMI. Results The median FFM(male: Z=74.74,female: Z=67.55) and FFMI(male: Z=56.13,female: Z=40.03) of both boys and girls increased with age(P<0.05 for both).The median FFM(Z=-23.33 to-2.32) and FFMI(Z=-21.24 to-3.46) of boys were higher than those of girls of the same age(P<0.05 for both),and the gap increased rapidly after the age of 11.The growth curves of FFM and FFMI in children and adolescents aged 3-17 in Suzhou were basically consistent with the growth trend of FFM and FFMI in children and adolescents aged 3-17 in East China.The median FFM of boys aged 5-17(Z=-13.47 to-2.71) and girls aged 4-17(Z=-9.48 to-2.82) in Suzhou, as well as the median FFMI of boys aged 5 and 7-17(Z=-8.76 to-5.00) and girls aged 3-7,11,13-15,and 17(Z=-4.90 to-2.15) were all higher than those in East China(P<0.05 for all). Conclusions There are certain differences in the growth of FFM and FFMI among children and adolescents of different nutritional conditions and genders, and the differences in FFM and FFMI between boys and girls increase rapidly after the age of 11.The growth curves of FFM and FFMI in children and adolescents aged 3-17 in Suzhou are basically consistent with the growth trend in East China, and the median FFM and FFMI are generally higher than those in East China.
