苏州市3~17岁儿童青少年身高体重和体质量指数分布
Distribution of height, weight and body mass index of children and adolescents aged 3-17 in Suzhou City
【摘要】 目的 探讨苏州市3~17岁儿童青少年身高、体重、体质量指数(BMI)分布特征,为区域儿童肥胖精准防控提供循证依据。方法 基于2023—2024年苏州市学生常见病及健康影响因素监测项目,采用多阶段分层整群随机抽样方法,抽取苏州市4个区县32所学校的3~17岁学生,最终纳入8 513人进行学生常见病及健康影响因素监测。应用Jonckheere-Terpstra检验分析身高、体重、BMI随年龄变化趋势,应用Wilcoxon秩和检验分析同年龄组性别间差异,采用Kruskal-Wallis H秩和检验比较同性别年龄组差异。采用GAMLSS模型拟合身高、体重、BMI百分位数曲线。结果 苏州市3~17岁男生和女生身高中位数分别介于101.8~175.5 cm和100.9~162.9 cm,体重中位数分别介于16.9~71.6 kg和16.5~57.7 kg, BMI中位数分别介于16.3~23.3和16.2~21.7。男生12岁时身高、体重增幅最大(7.8 cm、6.0 kg),女生约10岁时身高、体重增幅最大(7.2 cm、5.4 kg)。苏州市3~17岁儿童青少年身高、体重中位数均高于华东地区,男生7~17岁时身高差值均>1 cm,差值峰值为14岁(4.5 cm);女生6~17岁差值均>1 cm,差值峰值为16岁(3.8 cm);男、女生体重中位数均高于华东地区1 kg以上,17岁时差值最大,分别为9.4、4.7 kg。男生脂肪重聚(AR)年龄为4岁,早于华东地区1岁;女生AR年龄为6岁,晚于华东地区2岁。结论 苏州3~17岁学生身高、体重持续增长,高于华东地区水平,男生发生肥胖的风险更大,需重点关注。
【Abstract】 Objective To explore the distribution characteristics of height, weight, and body mass index(BMI) in children and adolescents aged 3-17 in Suzhou City, so as to provide evidence-based support for precise prevention and control of childhood obesity in thisarea. Methods Based on the monitoring project data of common diseases and health influencing factors among students in Suzhou City from 2023 to 2024,a multi-stage stratified cluster random sampling method was used to select students aged 3-17 from 32 schools in 4 districts and counties of Suzhou City, anda total of 8 513 students were included for monitoring of common diseases and health influencing factors among students.Jonckheere Terpstra test was used to analyze the trends of height, weight, and BMI with age, Wilcoxon rank-sum test was used to analyze gender differences within the same age group, and Kruskal Wallis H rank-sum test was used to compare age group differences within the same gender group.The GAMLSS model was used to fit the percentile curves of height, weight, and BMI. Results The median height of male and female students aged 3-17 in Suzhou city ranged from 101.8-175.5 cm and 100.9-162.9 cm respectively.The median weight of them ranged from 16.9-71.6 kg and 16.5-57.7 kg respectively.The median BMI of them ranged from 16.3-23.3 and 16.2-21.7 respectively.The height and weight of both male and female students continued to grow, with the largest increase in height and weight for boys at the age of 12(7.8 cm, 6.0 kg),and for girls at the age of around 10(7.2 cm, 5.4 kg).The median height and weight of children and adolescents aged 3-17 in Suzhou City continued to be higher than those in East China.The height difference for boys aged 7-17 was>1 cm, with the peak occurring at the age of 14 years old with the difference of 4.5 cm.For girls aged 6-17,the height difference was>1 cm, with the peak occurring at the age of 16 years old with the difference of 3.8 cm.The median weight of both boys and girls was over 1 kg higher than that in East China, with the highest difference occurring at the age of 17,which were 9.4 kg and 4.7 kg respectively.Adiposity rebound(AR) age for male students was 4 years old, 1 year earlier than that in East China, while AR age for female students was 6 years old, 2 years later than that in East China. Conclusions Students aged 3-17 in Suzhou continue to grow in height and weight, which is higher than the level in East China.Male students have a greater risk of obesity and need to be given special attention.
