【摘要】 目的 探讨左髂外静脉超声参数对左髂静脉压迫综合征(IVCS)的诊断价值。方法 选取2020年10月—2023年9月新泰市人民医院收治的64例拟行手术治疗的大隐静脉曲张患者。患者术前接受下肢深静脉造影，根据髂静脉受压程度分为左IVCS组(左侧髂静脉受压程度≥50%,n=36)和非IVCS组(左侧髂静脉受压程度<50%,n=28)。所有患者均接受髂外静脉超声参数检查，比较两组患者的相关参数，并进行多因素Logistic回归分析，绘制受试者工作特征(ROC)曲线评估左髂外静脉超声参数对左IVCS的诊断价值。结果 左IVCS组仰卧位内径指数比大于非IVCS组，内径变化、血流速度变化指数比小于非IVCS组，差异有统计学意义(P<0.05)；多因素Logistic回归分析结果显示，仰卧位内径指数比(OR=4.129)和内径变化指数比(OR=0.010)是左IVCS发生的独立影响因素(P<0.05)；绘制ROC曲线显示，联合检测较单一检测的曲线下面积更高(P<0.05)，联合检测的敏感度和特异度分别为0.829和0.765，仰卧位内径指数比检测的敏感度和特异度分别为0.795和0.681，内径变化指数比检测的敏感度和特异度分别为0.793和0.769。结论 仰卧位内径指数比和内径变化指数比等左髂外静脉超声参数联合检测对左IVCS的诊断价值较高。更多还原

【Abstract】 Objective To explore the diagnostic value of ultrasound parameters of the left external iliac vein for left iliac vein compression syndrome(IVCS).Methods Sixty-four patients with varicose great saphenous vein who were scheduled for surgical treatment in The People’ s Hospital of Xintai City from October 2020 to September 2023 were selected.Before the operation,they underwent lower extremity deep vein angiography.According to the degree of iliac vein compression,they were divided into the left IVCS group(left iliac vein compression degree≥ 50%,n=36) and the non-IVCS group(left iliac vein compression degree <50%,n=28).All patients underwent ultrasound parameter examination of the external iliac vein,and the relevant parameters of the two groups were compared,and multivariate Logistic regression analysis was performed.The receiver operating characteristic(ROC)curve was drawn to evaluate the diagnostic value of left external iliac vein ultrasound parameters for left IVCS.Results The internal diameter index in the supine position of the left IVCS group was larger than that of the non-IVCS group,and the indices of internal diameter change and blood flow velocity change were smaller than that of the non-IVCS group,and the differences were statistically significant(P <0.05);The results of multivariate Logistic regression analysis showed that the internal diameter index ratio in the supine position(OR=4.129) and the internal diameter change index ratio(OR=0.010) were independent influencing factors for left IVCS(P <0.05);The ROC curve drawing showed that the combined detection had a higher area under the curve than the single detection(P <0.05),the sensitivity and specificity of combined detection were 0.829 and 0.765,and the sensitivity and specifcity of the supine position internal diameter index ratio detection were 0.795 and 0.681,and the internal diameter change index ratio detection were 0.793 and 0.769.Conclusions The combined detection of internal diameter index ratio and internal diameter change index ratio of the left external iliac vein has a higher diagnostic value for left IVCS.更多还原
