【摘要】 目的 分析经尿道钬激光前列腺剜除术(HoLEP)治疗低龄前列腺增生的临床效果。方法 选取2021年4月—2024年1月玉林市红十字会医院泌尿外科收治的60例低龄(≤65岁)前列腺增生患者,根据治疗方式不同分为观察组和对照组各30例。观察组应用HoLEP治疗,对照组应用经尿道前列腺电切术(TURP)治疗。比较两组手术情况,手术前后国际前列腺症状评分(IPSS),术后3和6个月尿流动力学指标、炎症应激指标、性功能指标。结果 观察组膀胱冲洗时间短于对照组;术后3个月观察组IPSS评分低于对照组,膀胱残余尿量与最大尿流率高于对照组;术后3和6个月观察组的国际勃起功能指数问卷5项(IIEF-5)评分高于对照组,逆行射精率低于对照组,且观察组术后3个月与术前、术后6个月与术前、术后3个月与6个月IIEF-5评分差值高于对照组(P<0.05)。两组患者术前与术后12、24 h C反应蛋白水平比较,差异无统计学意义(P>0.05)。结论 低龄前列腺增生患者应用HoLEP治疗效果显著,在解除下尿路梗阻的同时保护性功能方面具独特的优势。更多还原

【Abstract】 Objective To analyze the effect of holmium laser enucleation of the prostate(HoLEP) in the treatment of younger patients with benign prostatic hyperplasia. Methods A total of 60 younger patients( ≤65 years old) with benign prostatic hyperplasia admitted to the Department of Urology, Red Cross Hospital of Yulin City from April 2021 to January 2024 were selected. According to different treatment methods, they were divided into an observation group(30 cases) and a control group(30 cases). The observation group was treated with HoLEP, and the control group was treated with transurethral resection of the prostate(TURP). The surgical conditions of the two groups, the International Prostate Symptom Score(IPSS) before and after surgery, urodynamic indicators, inflammatory stress indicators, and sexual function indicators at 3 and 6 months after surgery were compared. Results The bladder irrigation time in the observation group was shorter than that in the control group. At 3 months after surgery, the IPSS score in the observation group was lower than that in the control group, while the post-void residual urine volume and the maximum urine flow rate were higher than those in the control group. At 3 and 6 months after surgery, the International Index of Erectile Function-5(IIEF-5) score in the observation group was higher than that in the control group, and the retrograde ejaculation rate was lower than that in the control group. Moreover, the difference of IIEF-5 score between 3 months after operation and before operation, 6 months after operation and before operation, 3 months after operation and 6 months after operation in the observation group was higher than that in the control group, the differences were statistically significant(P < 0.05). There was no significant difference in the C-reactive protein levels between the two groups of patients before surgery and at 12 and 24 hours after surgery(P > 0.05). Conclusions HoLEP has a significant therapeutic effect on younger patients with benign prostatic hyperplasia and has unique advantages in relieving lower urinary tract obstruction and protecting sexual function.更多还原
