磁共振弥散加权成像ADC值与子宫内膜癌分化程度及Ki-67表达的关系
The relationship between ADC value of diffusion weighted magnetic resonance imaging and the differentiation degree and Ki-67 expression of endometrial cancer
【摘要】 目的 分析磁共振弥散加权成像(diffusion-weighted imaging, DWI)中的表观弥散系数(apparent diffusion coefficient,ADC)值与子宫内膜癌(endometrial carcinoma, EC)分化程度及Ki-67表达水平之间的相关性。方法 回顾性分析2019年1月至2024年10月在什邡市人民医院接受治疗的56例EC患者的数据。所有患者在诊断时接受了标准化的MRI扫描,收集其临床、病理资料以及ADC值。同时对肿瘤进行Ki-67免疫组织化学染色以评估增殖指数,统计Ki-67表达情况,分析其与患者临床病理的关系。采用单因素方差分析(ANOVA)对比不同分化程度及Ki-67表达EC患者ADC值,Spearman分析EC患者与分化程度、Ki-67表达的相关性。结果 不同分化程度EC患者ADCmean、ADCmax对比差异有统计学意义(P<0.05);不同Ki-67表达EC患者ADCmean、ADCmax及ADCmin对比差异有统计学意义(P<0.05);ADC值与EC肿瘤分化程度及Ki-67表达之间呈负相关(P<0.001)。结论 DWI中的ADC值与Ki-67表达水平显著相关,低ADC值可反映肿瘤的高增殖活性及较差的分化程度,为EC的诊断和预后评估提供影像学参考。
【Abstract】 Objective To analyze the correlation between the apparent diffusion coefficient(ADC) value from diffusion-weighted imaging(DWI) and the differentiation degree as well as Ki-67 expression level in endometrial carcinoma(EC).Methods Retrospective analysis data of 56 patients with EC treated at Shifang People’s Hospital between January 2019 to October 2024 were collected. All patients underwent standardized MRI scans at diagnosis, and clinical, pathological data, and ADC values were recorded. Ki-67 immunohistochemical staining was performed to assess the proliferation index of the tumors. The Ki-67 expression in EC tissues was statistically analyzed, and its relationship with clinicopathological characteristics was evaluated. One-way analysis of variance(ANOVA) was used to compare ADC value among patients with different differentiation degrees and Ki-67 expression levels, and Spearman correlation analysis was performed to assess the relationship between ADC value and differentiation degree as well as Ki-67 expression.Results Significant differences were observed in ADCmean and ADCmax among patients with different differentiation degrees(P<0.05). Similarly, significant differences were found in ADCmean, ADCmax, and ADCmin among patients with different Ki-67 expression level(P<0.05). ADC value showed a negative correlation with tumor differentiation degree and Ki-67 expression(P<0.001).Conclusion ADC value in MRI is significantly correlated with Ki-67 expression level. Lower ADC value reflects higher tumor proliferative activity and poorer differentiation, providing imaging references for the diagnosis and prognosis assessment of EC.
