依达拉奉右莰醇联合踏车有氧训练对急性缺血性脑卒中神经功能、心功能与心肺耐力的影响
Effects of edaravone dextrorphan combined with treadmill aerobic training on neurological function, cardiac function and cardiopulmonary endurance in patients with acute ischemic stroke
【摘要】 目的 探讨依达拉奉右莰醇联合踏车有氧训练对急性缺血性脑卒中(AIS)患者神经功能、心功能与心肺耐力的影响。方法 前瞻性选取2024年1月~2024年6月于本院接受治疗的96例AIS患者进行研究,并采用随机数字表法分为对照组(48例)和联合组(48例)。对照组给予依达拉奉右莰醇治疗,联合组在此基础上加踏车有氧训练治疗,两组均连续治疗4周。比较两组疗效、神经功能[美国国立卫生研究院卒中量表(NIHSS)评分]、心功能[左心室射血分数(LVEF)、心脏指数(CI)、心排出量(CO)]、心肺耐力[运动持续时间(ED)、6 min步行实验(6MWT)、峰值氧耗量(VO2max)]、下肢Fugl-Meyer评定量表(FMA-LE)评分、Barthel指数(BI)评分以及不良反应。结果 治疗后,联合组的治疗总有效率为93.75%,高于对照组的治疗总有效率77.08%(χ2=5.352,P<0.05)。治疗后联合组的LVEF、CI、CO、ED、6MWT、VO2max以及治疗前后NIHSS评分、FMA-LE评分、BI评分差值均高于对照组[(63.26±5.86)vs.(58.74±5.37)、(2.98±0.46)L/(min·m2)vs.(2.56±0.42)L/(min·m2)、(4.72±0.36)L/min vs.(4.28±0.31)L/min、(8.57±1.24)min vs.(7.64±1.08)min、(156.43±23.78)m vs.(135.27±18.84)m、(19.67±3.68)vs.(17.28±3.35)、(10.34±2.52)分vs.(6.37±1.83)分、(8.36±1.59)分vs.(4.72±1.14)分、(32.87±4.65)分vs.(24.26±3.54)分],差异均具有统计学意义(t=3.940、4.672、6.417、3.918、4.832、3.327、8.832、12.890、10.207,P<0.05)。治疗期间两组不良反应发生率比较差异无统计学意义(χ2=0.136,P> 0.05)。结论 AIS患者联合使用依达拉奉右莰醇与踏车有氧训练治疗的疗效显著,能有效改善心功能与心肺耐力,提升神经功能、下肢运动能力以及日常生活能力,值得临床应用。
【Abstract】 Objective [Abstract] Objective To investigate the effect of edaravone dextrose combined with treadmill aerobic training on neurological function, cardiac function and cardiopulmonary endurance in patients with acute ischemic stroke(AIS). Methods A total of 96 AIS patients who were treated in our hospital from January 2024 to June 2024 were prospectively selected for the study, and were randomly assigned to control group(n=48)and combined group(n=48). The control group was treated with edaravone right camphor, and the combined group was treated with treadmill aerobic training on this basis. Both groups were received 4-week continuous treatment. The efficacy, neurological function[ National Institutes of Health Stroke Scale(NIHSS)score ],cardiac function[ left ventricular ejection fraction(LVEF),cardiac index(CI),cardiac output(CO)],cardiopulmonary endurance[ exercise duration(ED),6-minute walking test(6MWT),peak oxygen consumption(VO2max)],lower limb Fugl-Meyer Assessment Scale(FMA-LE)score, Barthel Index(BI)score and adverse reactions were compared between the two groups. Results After treatment, the total effective rate of the combined group was 93.75 %,which was higher than that of the control group(77.08 %)(χ2=5.352,P < 0.05). After treatment, the LVEF,CI,CO,ED,6 MWT,VO2 max, NIHSS score, FMA-LE score and BI score in the combined group were higher than those in the control group, the differences were statistically significant(t=3.940,4.672,6.417,3.918,4.832,3.327,8.832,12.890,10.207,P < 0.05). There was had no difference in the incidence of side effect between the two teams(χ2=0.136,P > 0.05). Conclusion The combined use of edaravone right camphor and treadmill aerobic training in AIS patients demonstrates superior efficacy, which can effectively improve cardiac function and cardiopulmonary endurance, improve neurological function, lower limb motor ability and daily living ability, and may serve as a recommended clinical regimen.
