小剂量奥氮平联合多奈哌齐对阿尔茨海默病患者临床症状及ADAS-cog评分的影响

【摘要】 目的 观察小剂量奥氮平联合多奈哌齐对阿尔茨海默病患者临床症状的作用,以及对阿尔茨海默病评价量表-认知量表(Alzheimer’s disease assessment scale-cognitive scale, ADAS-cog)评分产生的影响。方法 按照随机数字表法将2021年5月至2023年5月于新乡医学院第三附属医院就诊的100例阿尔茨海默病患者分为对照组和观察组,每组50例。对照组给予多奈哌齐治疗,观察组在对照组基础上给予小剂量奥氮平。采用ADAS-cog、阿尔茨海默病病理行为评分量表(Alzheimer’s disease pathological behavior rating scale, BEHAVE-AD)及临床痴呆评定量表(clinical dementia rating scale, CDR)评价;检测血清乙酰胆碱酯酶(acetylcholinesterase, AchE)、乙酰胆碱(acetylcholine, Ach)、脑源性神经营养因子(brain-derived neurotrophic factor, BDNF)水平;比较临床疗效。结果 治疗后,观察组的ADAS-cog、BEHAVE-AD、CDR评分较对照组明显降低,差异有统计学意义(P<0.05);观察组AchE含量低于对照组,Ach、BDNF含量高于对照组,差异有统计学意义(P<0.05);观察组总有效率高于对照组,差异有统计学意义(P<0.05)。结论 小剂量奥氮平联合多奈哌齐治疗阿尔茨海默病可提升Ach、BDNF水平,减少AchE含量,降低ADAS-cog、BEHAVE-AD、CDR评分,改善临床症状及提升临床疗效。

【Abstract】 Objective To observe the effects of low-dose olanzapine combined with donepezil on clinical symptoms and scores of Alzheimer’s disease assessment scale-cognitive scale(ADAS-cog) in patients with Alzheimer’s disease.Methods Totally 100 patients with Alzheimer’s disease treated in the Third Affiliated Hospital of Xinxiang Medical University from May 2021 to May 2023 were selected, and all patients were divided into control group and obervation group by random number table method. The control group(50 cases) were given donepezil treatment while the observation group(50 cases) were given low-dose olanzapine on the basis of the control group.The ADAS-cog, Alzheimer’s disease pathological behavior rating scale(BEHAVE-AD), and clinical dementia rating scale(CDR) were evaluated. The serum acetylcholinesterase(AchE), serum acetylcholine(Ach) and brain-derived neurotrophic factor(BDNF) levels were detected before and after treatment, and the clinical efficacy was compared.Results The scores of ADAS-cog, BEHAVE-AD and CDR in observation group were lower than that in the control group, and the difference were ststistically significant(P<0.05). The AchE content of observation group was lower than the control group, while the Ach, BDNF contents of observation group were higher than those in the control group, and the difference were ststistically significant(P<0.05).The total effective rate of the observation group was higher than that in the control group(P<0.05).Conclusion Low-dose olanzapine combined with donepezil in the treatment of Alzheimer’s disease can improve the levels of Ach and BDNF, reduce the AchE content, reduce the scores of ADAS-cog, BEHAVE-AD and CDR, improve the clinical symptoms of patients and enhance the clinical efficacy.
