【摘要】 目的 旨在观察超声引导下肩关节腔臭氧加压注射联合Mulligan关节松动术治疗粘连性肩关节囊炎的临床效果。方法 收集2023年1月至2024年6月新乡医学院第一附属医院康复医学科就诊的56例粘连性肩关节囊炎患者，通过随机数字法将其分为两组:关节训练组和加压联合组，每组28例患者。两组患者均给予常规康复治疗，关节训练组给予Mulligan关节松动术，每天1次，每周5 d，连续治疗2周;加压联合组在关节训练组治疗基础上给予患侧肩关节腔内臭氧加压注射，浓度20 mg·L^-1，注射至推注时感到阻力时再继续注射5 m L，每周1次，治疗4周。给予疼痛视觉模拟评分（VAS）、肩关节活动度、Constant-Murley肩关节功能评分评价肩关节疼痛情况、活动范围及肩关节功能。检测血清中白细胞介素-6（IL-6）水平，评价粘连性肩关节囊炎的炎症情况。结果 组内比较，两组患者治疗后在VAS、肩关节活动度以及Constant-Murley评分均有改善（P<0.05），且两组治疗后IL-6水平均有降低（P<0.05）。治疗后组间比较，加压联合组的VAS、肩关节活动度、Constant-Murley评分均较关节训练组改善（P<0.05），加压联合组IL-6水平较关节训练组降低更明显（P<0.05）。结论 超声引导下肩关节腔臭氧加压注射联合关节松动术治疗可以缓解粘连性肩关节囊炎患者患侧肩关节疼痛，改善肩关节活动度和肩关节功能。更多还原

【Abstract】 Objective To explore the clinical effect of ultrasound-guided shoulder cavity ozone compression injection combined with Mulligan joint mobilization in the treatment of adhesive capsulitis.Methods From January 2023 to June 2024,56 patients with adhesive capsulitis in the Department of Rehabilitation Medicine of the First Affiliated Hospital of Xinxiang Medical University were randomly divided into two groups:joint training group and compression combined group,with 28 patients in each group.Both groups were given routine rehabilitation therapy,and the joint training group was given Mulligan joint mobilization once a day,5 days a week for 2 weeks.In the compression combined group,on the basis of the treatment of joint training group,ozone was injected into the affected shoulder joint cavity with a concentration of 20 mg·L^-1.When the injection was resisted, another 5 mL was injected once a week for 4 weeks.The visual analogue scale （VAS） score,shoulder range of motion,and Constant-Murley shoulder function score were used to evaluate shoulder pain,range of motion,and shoulder function.Serum interleukin-6 （IL-6） level was measured to evaluate the inflammation of adhesive capsulitis.Results Intra-group comparison:after treatment,both groups of patients showed improvements in VAS,shoulder range of motion,and ConstantMurley score （P<0.05）,and the IL-6 level decreased in both groups after treatment （P<0.05）.After treatment,when comparing between groups,the VAS,shoulder range of motion,and Constant-Murley score in the compression combined group were all improved compared with those in the joint training group （P<0.05）,and the IL-6 level in the compression combined group decreased more significantly than that in the joint training group （P<0.05）.Conclusion Ultrasound-guided intra-articular ozone compression injection combined with joint mobilization can relieve shoulder pain,improve shoulder mobility and shoulder function in patients with adhesive capsulitis.更多还原
