【摘要】 目的 探讨保留Retzius间隙的经腹腹腔镜根治术(LRS)治疗前列腺癌患者的效果。方法 选取2021年1月至2023年8月新乡医学院第一附属医院收治的84例前列腺癌患者，按随机数字表法分为保留间隙组和常规手术组，各42例。常规手术组行常规经腹LRS，保留间隙组行保留Retzius间隙的经腹LRS。比较两组围手术期指标、手术前后最大尿流率、性功能及并发症。结果 保留间隙组手术时间长于常规手术组，术中出血量、术后引流时间、术后导尿时间、住院时间短于常规手术组，术后并发症发生率低于常规手术组，术后6个月最大尿流率、国际勃起功能指数问卷表-5评分高于常规手术组(P<0.05)。两组切缘阳性率比较，差异无统计学意义(P > 0.05)。结论 保留Retzius间隙的经腹LRS在前列腺癌临床治疗中可促进术后恢复，减少尿控功能损伤、性功能障碍。更多还原

【Abstract】 Objective To investigate the surgical effect of transabdominal laparoscopic radical surgery( LRS) with Retzius space preservation in patients with prostate cancer. Methods A total of 84 patients with prostate cancer in the First Affiliated Hospital of Xinxiang Medical University from January 2021 to August 2023 were selected and divided into reserved gap group and routine surgery group by the random number table method,42 patients in each group. The routine surgery group underwent routine transabdominal LRS,and the reserved gap group underwent transabdominal LRS with Retzius space. Perioperative indicators,maximum urine flow rate,sexual function before and after surger,complications were compared between reserved gap group and routine surgery group. Results The operation time in the reserved gap group was longer than routine surgery group,the intraoperative blood loss,postoperative drainage time,postoperative catheterization time and hospitalization time were less than routine surgery group,the incidence of complications was lower than routine surgery group,the maximum urine flow rate and the International Index of Erectile Function Questionnaire-5 score after 6 months of surgery were higher than routine surgery group( P <0. 05). The positive rate of surgical margin had no difference between two groups( P >0. 05). Conclusion Transperitoneal LRS with Retzius space preservation can promote postoperative recovery in the clinical treatment of prostate cancer,reduce urinary control function damage and sexual dysfunction.更多还原
