运用PCNE分类系统对神经内科医嘱审核的效果评价
Evaluation of the Effect of PCNE Classification System on the Review of Medical Order in Neurology
【摘要】 目的 运用欧洲医药保健网(PCNE)分类系统,分析神经内科患者的药物相关问题(DRPs),为药师进行医嘱审核提供理论依据。方法 调取2023年4月~6月新疆医科大学附属中医医院审核系统中,神经内科药物相关性问题医嘱,根据PCNE分类系统,将DRPs进行分类、分析,最终对比信息系统审核和药师审核两种审核模式对医嘱的干预结果。结果 本研究纳入149例神经内科住院患者的药物相关问题医嘱,发生236个DRPs,其中药师审核组发生94个,系统审核组发生142个。DRPs问题集中在治疗安全性(71.61%),原因集中在药物选择(54.49%)和剂量选择不适宜(30.93%)。发生DRPs前三的药物为中枢神经系统、心血管系统、消化系统药物。信息化审核在选药不适宜(存在其他禁止)、相互作用、重复用药方面DRPs发现多于人工审核。比较两组审方对医嘱的干预结果,系统审核组的医嘱调整率为41.55%,药师审核组的医嘱调整率为77.66%,差异具有统计学意义(P<0.001)。结论 PCNE分类系统能够提高药师在审方中对DRPs的处理能力,并通过系统分类解决临床与审核系统存在的不适宜性问题,对促进临床合理用药起到更显著的作用。
【Abstract】 OBJECTIVE Based on the Pharmaceutical care network Europe(PCNE) classification system, analyze the drug-related problems(DRPs) of neurology patients, and provide a theoretical basis for pharmacists to review medical orders.METHODS The data of medical orders of patients admitted to the neurology department of the Affiliated Traditional Chineses Medicine Hospital of Xinjiang Medical University from April to June 2023 were retrieved, and drug-related problems(DRPs) were classified and analyzed according to the PCNE classification system to compare the results of interventions on medical orders by both systematic and manual auditing methods.RESULTS In this study, 149 inpatients in neurology were included in the medical orders with drug-related problems, and 236 DRPs occurred, including 94 in the pharmacist review group and 142 in the system review group.The problems of DRPs focused on the treatment safety(71.61%),and the reasons focused on drug selection(54.49%) and inappropriate dose selection(30.93%).The top three drugs with DRPs were the central nervous system drugs, cardiovascular system drugs and digestive system drugs.The information-based audit found more DRPs than the manual audits in terms of inappropriate drug selection(with other prohibitions),interactions, and repeated drug use.Comparing the intervention results of two groups of reviewing prescriptions, the adjustment rate of medical orders in the system review group was 41.55%,while that in the pharmacist review group was 77.66%.The difference was statistically significant(P<0.001).CONCLUSION The introduction of the PCNE classification system can improve the ability of pharmacists to solve DRPs into the real-time medication review system, and solve the inappropriateness of the clinical and review systems through systematic classification, which plays a more significant role in promoting clinical rational drug use.
