血清miR-483、miR-210、Klotho蛋白水平对尿毒症患者心血管并发症的预测价值分析

【摘要】 目的:探究血清微小核糖核酸(miR)-483、miR-210和Klotho蛋白水平对尿毒症患者心血管并发症的预测价值。方法:选取2019年1月至2023年2月我院收治的尿毒症患者60例,根据随访2年内是否发生心血管并发症,将其分成心血管并发症组(26例)与无心血管并发症组(34例),对比两组血清miR-483、miR-210和Klotho蛋白水平。通过多变量Logistic回归模型探究尿毒症患者发生心血管并发症的风险因素;利用受试者工作特征(ROC)曲线评估血清miR-483、miR-210和Klotho蛋白水平单独及联合检测对尿毒症合并心血管并发症患者的诊断效能。结果:心血管并发症组患者血清miR-483和miR-210水平均较非心血管并发症组高,而Klotho蛋白水平较非心血管并发症组低,差异有统计学意义(t=3.128、4.381、3.294,P<0.05);Logistic分析得出,年龄、血液透析、高血压、血红蛋白、LVMI、miR-483和miR-210是尿毒症患者发生心血管并发症的危险因素,而LVEF、Klotho蛋白是尿毒症患者发生心血管并发症的保护因素(P<0.05);血清miR-483、miR-210和Klotho蛋白以及三者联合检测的ROC曲线面积分别为0.701、0.806、0.734、0.869,联合预测优于单一预测(P<0.05)。结论:尿毒症合并心血管并发症患者的血清miR-483和miR-210呈高表达,Klotho蛋白呈低表达,三者联合检测对于尿毒症患者心血管并发症具有较高的预测价值。

【Abstract】 Objective:To research the potential of serum micro-Ribonucleic acid(miR)-483,miR-210 and Klotho protein quantities as biomarkers for cardiovascular incidents in individuals with uremia.Methods:Sixty individuals suffering from uremia were enrolled at our hospital during the period from January 2019 to February 2023.These individuals were examined over a period of 2 years and categorized into two groups,according to the occurrence of cardiovascular occurrences:the cardiovascular morbidity group(26 cases)and the group without cardiovascular morbidities(34 cases). The serum values of miR-483,miR-210,and Klotho protein were then juxtaposed amongst the two cohorts. The investigation of risk elements for cardiovascular incidents in patients with uremia was conducted utilizing multiple logistic models. The diagnostic efficacy of serum levels of miR-483,miR-210 and Klotho protein,both individually and in conjunction,in patients with uremia in conjunction with cardiovascular incidents was appraised by employing the subject’s work characteristics(ROC)curve.Results:Serum miR-483 and miR-210 levels were elevated in the cardiovascular morbidity group relative to the non-cardiovascular morbidity group,while Klotho protein levels were diminished relative to the non-cardiovascular morbidity group,and these differences were statistically significant(t=3.128,4.381,and 3.294,P<0.05). Logistic analyses indicated that age,haemodialysis,hypertension,haemoglobin,LVMI,miR-483 and miR-210 were risk elements for cardiovascular morbidity in patients with uremia,while LVEF and Klotho protein were protective elements for cardiovascular morbidity in subjects with uremia(P<0.05). The AUCs for serum miR-483,miR-210 and Klotho protein,and the composite of the three assays,were 0.701,0.806,0.734,and 0.869,respectively. The integrated prediction exhibited superiority over the standalone prediction(P<0.05).Conclusion:Serum expression of miR-483 and miR-210 was elevated in patients with uremia in conjunction with cardiovascular complications,while Klotho protein expression was diminished. The combination of the three assays displayed a high potential for predicting cardiovascular morbidity in individuals with uremia.
