铬酸盐职业暴露与机体免疫炎症的关联研究——基于OCEDCC的横断面调查
Association between occupational exposure to chromate and immune inflammation: a cross-sectional survey based on OCEDCC
【摘要】 目的 观察铬酸盐接触人群外周全血铬与免疫炎症标志物之间的相关性,明确可溶性铬酸盐职业接触对机体免疫炎症的影响,为铬酸盐类有害因素的健康监护提供参考。方法 依托铬酸盐接触人群动态队列(Occupational Chromate Exposure Dynamic Cohort of China,OCEDCC)选取首次纳入的职业接触者为接触组,同企业中无明显有害接触的人员为对照组。采用电感耦合等离子体质谱法测定各组研究对象外周全血铬含量。采用颗粒增强免疫透射比浊法和免疫透射比浊法检测外周血清高敏性C反应蛋白(Creactive protein,CRP)和血清补体C1q(complement component 1q,C1q)浓度。结果 共纳入1 295名研究对象(接触组688名,对照组607名)。两组在人口学特征上差异均无统计学意义(t=0.946、-1.115、1.113,P>0.05);接触组全血铬浓度(6.06±12.93)μg/L高于对照组(1.43±2.11)μg/L;接触组血清中CRP水平低于对照组(Z=1.838,P<0.05)。控制混杂因素后,人群全血铬与血清中C1q呈显著负相关。随全血铬升高,血清中CRP和C1q水平均呈先升后降趋势。结论 铬酸盐接触可以引起人群全血铬的显著增高以及血清免疫炎症指标的变化,相关的健康监护中推荐加强免疫炎症的监测。
【Abstract】 Objective To investigate the association between peripheral whole blood chromium levels and immunoinflammatory markers among chromate-exposed populations, clarify the effects of occupational exposure to soluble chromate on immunoinflammatory responses,and to provide references for the health monitoring of chromaterelated hazards.Methods Participants were selected from the Occupational Chromate Exposure Dynamic Cohort of China(OCEDCC), with first-time occupationally exposed individuals assigned to the exposure group and workers without significant hazardous exposures from the same enterprises serving as the control group.Peripheral whole blood chromium concentrations were measured using inductively coupled plasma mass spectrometry(ICP-MS). Serum concentrations of high-sensitivity C-reactive protein(CRP)and complement component 1q(C1q)were determined using particle-enhanced immunoturbidimetric assays and immunoturbidimetric assays,respectively.Results A total of 1295 participants were included(688 in the exposure group and 607 in the control group).There were no statistically significant differences in demographic characteristics between the two groups(t=0.946,-1.115,1.113,P>0.05).The mean whole blood chromium concentration was significantly higher in the exposure group(6.06±12.93) μg/L than that in the control group(1.43±2.11) μg/L. Serum CRP levels were significantly lower in the exposure group compared to the control group(Z=1.838,P<0.05). After adjusting for potential confounders,whole blood chromium levels showed a significant negative correlation with serum C1q concentrations.As whole blood chromium levels increased, both CRP and C1q levels exhibited an initial rise followed by a decline.Conclusion Exposure to chromate can cause a significant increase in whole blood chromium levels and changes in serum immune inflammatory markers in the population. It is recommended to strengthen the monitoring of immune inflammation in related health monitoring.
