外周血SDC-1、MDA-LDL与血液透析患者动脉硬化的相关性

【摘要】 目的 探讨外周血多配体蛋白聚糖-1(SDC-1)、丙二醛修饰的低密度脂蛋白(MDA-LDL)与血液透析患者动脉硬化(AS)的相关性及其临床预测价值。方法 回顾性选取2022年1月至2024年6月该院收治的125例血液透析患者作为血液透析组,另选取同期来院体检的65例健康者作为对照组。根据是否发生AS将血液透析患者分为AS组(65例)和无AS组(60例)。采用酶联免疫吸附试验检测外周血SDC-1、MDA-LDL水平。通过Spearman相关分析血液透析患者外周血SDC-1、MDA-LDL水平与肱踝脉搏波速度(baPWV)的相关性,多因素非条件Logistic回归确定外周血SDC-1、MDA-LDL水平与血液透析患者AS的关系,受试者工作特征(ROC)曲线分析外周血SDC-1、MDA-LDL水平对血液透析患者AS的预测效能。结果 与对照组比较,血液透析组外周血SDC-1、MDA-LDL水平升高(P<0.05)。血液透析患者外周血SDC-1、MDA-LDL水平与左baPWV、右baPWV呈正相关(P<0.05)。SDC-1高、MDA-LDL高均为血液透析患者AS的独立危险因素(P<0.05)。外周血SDC-1、MDA-LDL水平联合预测血液透析患者AS的曲线下面积为0.858,大于外周血SDC-1、MDA-LDL水平单独预测的0.785、0.778(P<0.05)。结论 血液透析患者外周血SDC-1、MDA-LDL水平升高与AS密切相关,外周血SDC-1、MDA-LDL水平联合检测对AS的预测效能较高。

【Abstract】 Objective To investigate the correlation between peripheral blood syndecan-1(SDC-1) and malondialdehyde-modified low-density lipoprotein(MDA-LDL) with arteriosclerosis(AS) in hemodialysis patients and their clinical predictive value.Methods A total of 125 hemodialysis patients admitted to the hospital from January 2022 to June 2024 as the hemodialysis group, and 65 healthy individuals who underwent physical examination during the same period as the control group.According to whether AS occurred, hemodialysis patients were divided into AS group(65 cases) and non-AS group(60 cases).The levels of SDC-1 and MDA-LDL in peripheral blood were detected by enzyme-linked immunosorbent assay.The correlation between the levels of SDC-1 and MDA-LDL in peripheral blood of hemodialysis patients and brachial-ankle pulse wave velocity(baPWV) was analyzed by Spearman correlation analysis.Multivariate unconditional Logistic regression was used to determine the relationship between the levels of SDC-1 and MDA-LDL in peripheral blood and AS in hemodialysis patients.The receiver operating characteristic(ROC) curve was used to analyze the predictive efficacy of peripheral blood SDC-1 and MDA-LDL levels for AS in hemodialysis patients.Results Compared with the control group, the levels of SDC-1 and MDA-LDL in the peripheral blood of the hemodialysis group increased(P<0.05).The levels of SDC-1 and MDA-LDL in the peripheral blood of hemodialysis patients were positively correlated with left baPWV and right baPWV(P<0.05).High SDC-1 and high MDA-LDL were both independent risk factors for AS in hemodialysis patients(P<0.05).The area under the curve for the combined prediction of AS in hemodialysis patients by peripheral blood SDC-1 and MDA-LDL levels was 0.858,which was greater than 0.785 and 0.778 predicted separately by peripheral blood SDC-1 and MDA-LDL levels(P<0.05).Conclusion The increase of SDC-1 and MDA-LDL levels in peripheral blood of hemodialysis patients is closely related to AS,and the combination of SDC-1 and MDA-LDL levels in peripheral blood has a higher energy efficiency in predicting AS.
