【Abstract】 Conventional treatments for delayed union or nonunion after fractures are often associated with long treatment cycles and significant financial burdens for patients. In recent years, cell-based therapies that promote fracture healing have emerged as promising strategies, attracting widespread attention and research interest. This review summarizes recent advances in the role of macrophage-derived exosomes(MD-Exos) in mediating osteogenesis and angiogenesis to promote fracture healing. The review focuses on how MD-Exos facilitate angiogenesis, repair vascular injury, upregulate osteogenesis-related factors, modulate bone marrow mesenchymal stem cells, and exert beneficial effects through various biological products. By exploring the potential relationship between MDEexos and fracture healing, this article aims to provide new insights and therapeutic perspectives for clinical strategies to enhance bone repair and treat bone nonunion.
