吸入麻醉药对肾脏缺血再灌注损伤的保护作用

【摘要】 肾脏缺血再灌注损伤(Ischemia-reperfusion injury,IRI)是造成急性肾损伤(Acute kidney injury,AKI)的主要病因,常发生于移植、大血管、肝脏、心脏等危重手术中。近年来,较多研究发现吸入麻醉药能减轻肾脏IRI,对围术期肾脏功能保护可能有着较好的应用前景。吸入麻醉药对肾脏的保护作用可能与其处理时机、处理浓度及作用时间窗有关。激活肾脏内多条信号通路发挥抗氧化、抗炎、抗凋亡作用以及调控基因表达和参与免疫调节可能是吸入麻醉药改善肾脏IRI的主要机制。本文结合近年来国内外研究文献,对吸入麻醉药减轻肾脏IRI、保护肾脏功能的动物实验和临床研究及其作用机制进行综述,旨在为围术期肾功能保护提供参考。

【Abstract】 Renal ischemia-reperfusion injury(IRI) is the main cause of acute kidney injury(AKI), often occurring during critical surgeries such as transplantation, large vessel surgery, liver surgery, and heart surgery. In recent years, many studies have found that inhalation anesthetics can alleviate renal IRI and may have good prospects for perioperative protection of renal function. The protective effect of inhalation anesthetics on the kidneys may be related to the timing, concentration, and time window of treatment. Antioxidative, anti-inflammatory and anti-apoptotic effects by activating multiple signaling pathways in the kidney, regulating gene expression and immunoregulation may be the main mechanism by which inhalation anesthetics improve renal IRI. This article provides a review of experimental and clinical studies on the use of inhalation anesthetics to alleviate renal IRI and protect renal function, as well as their mechanisms of action, based on recent domestic and international research literature. The aim is to provide reference for perioperative renal function protection.
