多普勒超声评估高危妊娠胎儿静脉导管血流动力学及其对宫内缺氧的预测价值
Doppler Ultrasound Assessment of Fetal Venous Catheter Hemodynamics in High-risk Pregnancy and its Predictive Value for Intrauterine Hypoxia
【摘要】 目的:探讨多普勒超声评估高危妊娠胎儿静脉导管血流动力学及预测宫内缺氧的价值。方法:选取2020年1月至2022年1月中晚期孕妇112例,分为高危妊娠组(62例)和正常组(50例)。多普勒超声检测静脉导管血流动力学参数(PS、ED、A)及大脑中动脉、脐动脉血流指标。根据宫内缺氧情况,高危妊娠组进一步分为观察组(32例)和对照组(30例)。结果:高危妊娠组PS[(47.86±6.84)cm/s]、ED[(43.71±6.12)cm/s]、A[(25.79±5.48)cm/s]显著高于正常组(P<0.05)。观察组大脑中动脉S/D值(3.06±0.62)低于对照组(4.61±0.73),脐动脉S/D值(4.01±0.50)高于对照组(2.64±0.47)(P<0.05)。ROC曲线分析显示,大脑中动脉和脐动脉血流指标预测宫内缺氧的AUC值分别为0.957和0.980(P<0.05)。结论:多普勒超声能有效评估高危妊娠胎儿静脉导管血流动力学变化,结合大脑中动脉和脐动脉血流监测,可准确预测宫内缺氧,对高危妊娠管理具有重要指导意义。
【Abstract】 Objective:To explore the value of Doppler ultrasound in evaluating the hemodynamics of fetal venous catheter in high-risk pregnancy and predicting intrauterine hypoxia. Methods: 112 pregnant women from January 2020 to January 2022 were selected and divided into high-risk pregnancy group(62 cases) and normal group(50 cases). Doppler ultrasound was used to detect hemodynamic parameters(PS, ED, A) of venous catheter and blood flow indexes of middle cerebral artery and umbilical artery. According to intrauterine hypoxia, the high-risk pregnancy group was further divided into observation group(32 cases) and control group(30 cases).Results: PS[(47.86±6.84) cm/s], ED[(43.71±6.12) cm/s] and A [(25.79±5.48) cm/s] in high-risk pregnancy group were significantly higher than those in normal group(P<0.05). The S/D value of middle cerebral artery(3.06±0.62) in the observation group was lower than that of the control group(4.61±0.73), and the S/D value of umbilical artery(4.01±0.50) was higher than that of the control group(2.64±0.47)(P<0.05). ROC curve analysis showed that the AUC values predicted by blood flow indexes of middle cerebral artery and umbilical artery were 0.957 and 0.980, respectively(P<0.05). Conclusion: Doppler ultrasound can effectively evaluate the hemodynamic changes of fetal venous catheter in high-risk pregnancy, and combined with blood flow monitoring of middle cerebral artery and umbilical artery, it can accurately predict intrauterine hypoxia, which has important guiding significance for high-risk pregnancy.
