【摘要】 目的:采用心脏磁共振（CMR）mapping技术评估抗MDA5抗体阳性皮肌炎患者的左、右心室功能损害情况。方法:前瞻性纳入20例抗MDA5抗体阳性皮肌炎患者和20名健康对照者。所有参与者均接受CMR扫描，测量左、右心室功能参数，左心室心肌及左、右心室的T₁和T₂值、右心室长轴应变（RV-LAS）以及右心室重构指数（RVESRI）。采用独立样本t检验和Pearson相关分析进行统计学分析。结果:抗MDA5抗体阳性皮肌炎患者的左心室心肌T₁及T₂值显著高于健康对照组（P<0.05）。与健康对照组相比，抗MDA5抗体阳性皮肌炎患者的左、右室功能显著降低（P<0.05）,左、右心室腔血池T₁值及其比值、右心室收缩末期容积指数（RVESVI）、RVESRI及RV-LAS均显著升高（P<0.05）。结论:CMR结果提示抗MDA5抗体阳性皮肌炎患者存在亚临床心脏损害。CMR可能为无创评估抗MDA5抗体阳性皮肌炎患者心脏并发症提供重要信息。更多还原

【Abstract】 Objective:To evaluate left and right ventricular functional impairment in patients with anti-MDA5 antibody-positive dermatomyositis（anti-MDA5 Ab+ DM） using cardiac magnetic resonance（CMR） imaging.Methods:This prospective study included 20 patients with anti-MDA5 Ab+ DM and 20 healthy controls.All participants underwent CMR scanning to measure left and right ventricular functional parameters, T₁ and T₂ values of the left ventricular myocardium as well as the left and right ventricles, right ventricular long-axis strain（RV-LAS）,and right ventricular end-systolic remodeling index（RVESRI）.Statistical analysis was performed using independent samples t-test and Pearson correlation analysis.Results:The left ventricular myocardial T₁ and T₂ values in patients with anti-MDA5 Ab+ DM were significantly higher than those in the healthy controls（P<0.05）.Compared with healthy controls, patients with anti-MDA5 Ab+ DM showed significant reductions in both left and right ventricular function（P<0.05）,along with significant increases in the T₁ values of the left and right ventricular cavity blood pools and their ratio, right ventricular end-systolic volume index（RVESVI）,RVESRI and RV-LAS（P<0.05）.Conclusion:CMR findings suggest the presence of subclinical cardiac impairment in patients with anti-MDA5 antibody-positive dermatomyositis.CMR may provide important information for the non-invasive assessment of cardiac complications in patients with anti-MDA5 antibody-positive dermatomyositis.更多还原
