【摘要】 目的:探讨内环口缩窄成型技术在腹腔镜经腹腹膜前疝修补术治疗男性Ⅲ型腹股沟斜疝中的应用价值。方法:回顾分析2023年1月至2023年12月接受腹腔镜经腹腹膜前疝修补术的84例男性Ⅲ型腹股沟斜疝患者的临床资料。根据斜疝内环口缺损的处理方式分为两组，40例采用内环口缩窄成型术(应用3-0倒刺缝线缝合缩窄内环口至≤1 cm，缩窄组)，44例常规处理(常规组)。结果:84例患者均顺利完成腹腔镜手术。两组患者临床基线资料、术中出血量、住院费用及术后住院时间差异无统计学意义;缩窄组手术时间长于常规组(t=4.843,P<0.001)，术后血清肿发生率低于常规组(5%vs. 22.7%，χ~2=5.377,P=0.020)。结论:腹腔镜经腹腹膜前疝修补术治疗成年男性Ⅲ型腹股沟斜疝术中采用内环口缩窄成型是安全、可靠的，能有效预防血清肿的发生，值得推广应用。更多还原

【Abstract】 Objective: To investigate the application value of the internal ring defect narrowing technique in laparoscopic transabdominal preperitoneal hernia repair(TAPP) for type Ⅲ indirect inguinal hernia in adult males.Methods: The clinical data of 84 adult male patients with type Ⅲ indirect inguinal hernia who underwent TAPP from Jan. 2023 to Dec. 2023 were retrospectively analyzed.The patients were divided into two groups based on the management of the hernia ring defect: the narrowing group(n=40),in which the internal ring was narrowed to ≤1 cm using 3-0 barbed suture,and the conventional group(n=44),which received no specific treatment of the hernia ring. Results: All operations were completed successfully. There were no significant differences in baseline c linical data,intraoperative blood loss,hospitalization costs,or postoperative hospital stay between the two groups. The operation time in the narrowing group was longer than that in the conventional group(t=4.843,P<0.001).The incidence of postoperative seroma in the narrowing group was significantly lower than that in the conventional group(5% vs. 22.7%,χ~2= 5.377,P=0.020).Conclusions: The internal ring defect narrowing technique in laparoscopic TAPP for type Ⅲ indirect inguinal hernia in adult males is safe and reliable. It can effectively reduce the incidence of seroma and is worthy of widespread clinical application.更多还原
