振幅整合脑电图评分系统在颅内出血新生儿中的应用

【摘要】 目的 探讨振幅整合脑电图(aEEG)评分系统在颅内出血新生儿中的应用。方法 回顾性分析2023年1月至2024年3月广西医科大学第一附属医院新生儿科收治的85例颅内出血新生儿,根据Papile影像学分度标准分为轻度出血组(Ⅰ/Ⅱ级,79例)和中重度出血组(Ⅲ/Ⅳ级,6例),对两组aEEG的结果进行比较分析。结果 中重度出血组睡眠觉醒周期(Cy)评分低于轻度出血组,差异有统计学意义(P<0.05)。aEEG的Cy评分与颅内出血严重程度呈负相关(r=-0.226,P=0.037),Cy评分越低,颅内出血越严重。结论aEEG的Cy改变与新生儿颅内出血严重程度有关,aEEG能早期诊断和预测新生儿颅内出血的严重程度。

【Abstract】 Objective To explore the application of amplitude integrated electroencephalogram(aEEG) score system in neonates with intracranial hemorrhage. Methods A total of 85 neonates with intracranial hemorrhage admitted to the Department of Neonatology, the First Affiliated Hospital of Guangxi Medical University from January 2023 to March 2024 were retrospectively analyzed. They were divided into mild hemorrhage group(grade Ⅰ/Ⅱ, 79 cases) and moderate to severe hemorrhage group(grade Ⅲ/Ⅳ, 6 cases) according to Papile image credit standard, and the results of aEEG in the two groups were compared and analyzed. Results The sleep wake cycle(Cy) score in the moderate to severe hemorrhage group was lower than that in the mild hemorrhage group(P < 0.05), and the difference was statistically significant. The Cy score of aEEG was negatively correlated with the severity of intracranial hemorrhage(r=-0.226,P=0.037). The lower the Cy score, the more severe the intracranial hemorrhage. Conclusion The Cy change of aEEG is related to the severity of neonatal intracranial hemorrhage. aEEG can diagnose and predict the severity of neonatal intracranial hemorrhage at an early stage.
