非增强Dixon水脂分离CS-SENSE全心CMRA技术在冠状动脉狭窄诊断中的价值分析
Value of Unenhanced Dixon Water-fat Separation CS-SENSE Whole-heart CMRA Technique in the Diagnosis of Coronary Artery Stenosis
【摘要】 目的 分析非增强Dixon水脂分离CS-SENSE全心CMRA技术在冠状动脉狭窄诊断中的价值。方法 回顾性分析2022年11月至2024年11月收治的104例疑似冠状动脉狭窄患者的临床资料,将冠状动脉造影检查作为诊断金标准,分析非增强Dixon水脂分离CS-SENSE全心CMRA技术对冠状动脉狭窄的诊断效能;同时分析对不同狭窄严重程度的诊断效能。结果 进行非增强Dixon水脂分离CS-SENSE检测的扫描时间短于传统2D SENSE技术(P<0.05),图像质量得分高于传统2D SENSE技术(P<0.05),心率和冠状动脉总长度差异无统计学意义(P>0.05);104例疑似冠状动脉狭窄患者经冠状动脉造影确诊阳性病例71例,阴性病例33例,非增强Dixon水脂分离CS-SENSE全心CMRA技术和传统2D SENSE全心CMRA的Kappa值分别为0.802、0.651;经冠状动脉造影可知,动脉狭窄程度50~69%的有54例,≥70%的患者有17例,非增强Dixon水脂分离CS-SENSE全心CMRA技术和传统2D SENSE全心CMRA对不同狭窄程度诊断的准确度分别为92.96%[(51+15)/71]和85.92%[(48+13)/71]。结论 非增强Dixon水脂分离CS-SENSE全心CMRA技术可参与临床中疑似冠状动脉狭窄患者的诊断,无创、无辐射,对冠状动脉狭窄诊断性能高,可以在保证图像质量的情况下大幅缩短采集时间。
【Abstract】 Objective To analyze the value of unenhanced Dixon water-fat separation CS-SENSE wholeheart CMRA technique in the diagnosis of coronary artery stenosis.Methods The clinical data of 104 patients with suspected coronary artery stenosis admitted from November 2022 to November 2024 were retrospectively analyzed.Coronary angiography was used as the gold standard for diagnosis,and the diagnostic efficiency of unenhanced Dixon water-fat separation CS-SENSE whole-heart CMRA technique on coronary artery stenosis was analyzed,and the diagnostic efficiency on different stenosis severities was analyzed.Results The scanning time of unenhanced Dixon wate r-fat separation CS-SENSE technique was shorter than that of tra ditional2D SENSE technique(P<0.05),and the image quality score was higher than that of traditional2D SENSE technique(P<0.05).There were no statistical differences in heart rate and total coronary artery length(P>0.05).There were 71 positive cases and 33 negative cases among 104patients with suspected co ronary artery stenosis confirmed by co ronary angiography.The Kappa values of unenhanced Dixon water-fat separation CS-SENSE whole-heart CMRA technique and traditional 2D SENSE whole-heart CMRA technique we re 0.802 and 0.651 respectively.Coronary angiography revealed that there were 54 patients with 50-69% stenosis and 17 patients with≥70% stenosis.The accuracy rates of unenhanced Dixon water-fat separation CS-SENSE wholeheart CMRA technique and traditional 2D SENSE whole-heart CMRA technique were 92.96%(66/71) and 85.92%(61/7)in the diagnosis of different stenosis degrees.Conclusion U nenhanced Dixon water-fat separation CS-SENSE whole-heart CMRA technique can participate in the diagnosis of patients with suspected coronary artery stenosis in clinical practice.It is noninvasive,non-ra diative,and has high diagnostic perfo rmance for coronary arte ry stenosis,and it can greatly shorten the acquisition time while ensuring image quality.
