【摘要】 目的 探讨分层递进式教学在住院医师规范化培训(简称住培）心电图判读教学中的优势及其教学效果。方法 以2024年7月至2024年12月在复旦大学附属华山医院参加住培的50名非心内科专业住院医师为研究对象，采用随机抽签的方法分为对照组和观察组各25人。两组均进行2课时的心电图知识回顾与梳理及2课时的五步读图法教学。对照组进行4课时的心电图实例讨论课，观察组依托心电图分级图库进行4课时的分层递进式教学。采用理论考试评价两组的心电图判读能力；采用问卷调查两组对课程的满意度及对读图能力的自我评价；采用迷你临床演练评估（Mini-CEX）量表评价两组的临床胜任力。结果 观察组理论考试的正确率、完整性高于对照组；观察组对心电图教学的满意度及自我评价高于对照组；观察组的Mini-CEX量表在沟通技能、临床判断、人文关怀及整体表现中得分高于对照组，以上差异均有统计学意义（均P<0.05）。结论 分层递进式教学有利于提高住院医师对心电图的判读能力，是值得推广的教学模式。更多还原

【Abstract】 Objective To explore the advantages and teaching effectiveness of the hierarchical progressive teaching model in the teaching of electrocardiogram(ECG) interpretation ability of residents during standardized residency training. Methods Fifty non-cardiology residents participating in the standardized residency training in our hospital from July to December 2024 were selected as the research subjects. They were randomly assigned to the control group(n= 25) and the experimental group(n= 25) by lottery. Both groups received a total of 2 class hours of ECG knowledge review and 2 class hours of 5-step ECG reading teaching. The control group had 4 class hours of ECG case discussion, while the experimental group carried out 4 class hours of hierarchical progressive teaching relying on the ECG grading image library. A theoretical examination was used to evaluate the ECG interpretation ability of the two groups. Questionnaires were used to investigate the satisfaction with courses and the self-evaluation of ECG reading ability of the two groups. The mini-clinical evaluation exercise(Mini-CEX) was used to evaluate the clinical competence of the two groups. Results The accuracy rate and integrity of the test in the experimental group were higher than those in the control group; The satisfaction with ECG teaching and the self-evaluation in the experimental group were higher than those in the control group; The experimental group scored higher than the control group in communication skills, clinical judgment, humanistic care, and overall performance rating items; The above differences were statistically significant(all P< 0.05). Conclusion The hierarchical progressive teaching is beneficial for improving residents’ ECG interpretation ability and is a teaching model worthy of promotion.更多还原
