【摘要】 目的：探讨基于脑电图的密度谱阵列图谱指导麻醉管理对行斜视矫治术的学龄前儿童苏醒期谵妄的影响。方法：选择在医院择期行斜视矫治术的80例学龄前患儿，采用随机数表法分为实验组和对照组，各40例。实验组患儿通过调节吸入七氟烷浓度，维持DSA图谱稳定的α和慢δ震荡；对照组患儿矫正年龄后，维持呼气末七氟烷肺泡浓度为1MAC。术中记录两组患儿麻醉维持期七氟烷浓度和PSI值，术后应用小儿麻醉苏醒期谵妄量表(PAED)评估两组患儿苏醒期谵妄(ED)发生情况，记录患儿苏醒时间、麻醉后监测治疗室(PACU)驻留时间。结果：实验组患儿的ED发生率低于对照组(P<0.05)；实验组患儿麻醉维持期七氟烷浓度显著低于对照组(P<0.05)，且实验组患儿PSI值明显高于对照组(P<0.05)；实验组患儿苏醒时间短于对照组(P<0.05)。结论：DSA图谱指导麻醉管理有助于降低择期行斜视矫治术的学龄前患儿的ED发生率，减少学龄前儿童术中七氟烷暴露。更多还原

【Abstract】 Objective:To investigate the effect of electroencephalogram-based density spectral array (DSA) spectrum guided anesthesia management on emergence delirium (ED) in preschool children undergoing strabismus correction surgery.Methods:Eighty preschool children scheduled for elective strabismus correction surgery in the hospital were selected and randomly divided into two groups using a random number table,with 40 cases in the experimental group and 40 cases in the control group.In the experimental group,the concentration of inhaled sevoflurane was adjusted to maintain stable α and slow δ oscillations on the DSA spectrum.In the control group,the end-tidal sevoflurane concentration was maintained at 1 MAC after age adjustment.The sevoflurane concentration and Patient State Index (PSI) values during the anesthesia maintenance period were recorded for both groups.Postoperatively,the Pediatric Anesthesia Emergence Delirium (PAED) scale was used to assess the incidence of ED in both groups.Recovery time and post-anesthesia care unit (PACU) stay duration were also recorded.Results:The incidence of ED in the experimental group was lower than that in the control group (P<0.05).The sevoflurane concentration during anesthesia maintenance in the experimental group was significantly lower than that in the control group (P<0.05),and the PSI values in the experimental group were significantly higher than those in the control group (P<0.05).Additionally,the recovery time in the experimental group was shorter than that in the control group (P<0.05).Conclusion:DSA spectrum-guided anesthesia management may help reduce the incidence of ED and decrease intraoperative sevoflurane exposure in preschool children undergoing elective strabismus correction surgery.更多还原
