激光光凝术后视网膜新生血管形成对患者视功能的影响研究

【摘要】 目的 探究激光光凝术后视网膜新生血管形成(RNV)对患者视功能的影响因素。方法 选取2021年1月至2023年7月在本院接受激光光凝术治疗的120例RNV患者纳入研究。测定患者治疗前1 d和术后3个月的最佳矫正视力(BCVA)以评估视功能,并根据术后3个月BCVA将患者分为视功能不良组和视功能良好组。比较两组患者的临床资料,并对相关因素进行多因素分析。结果 患者术后3个月的BCVA显著高于治疗前1 d(P<0.05)。视功能不良组和视功能良好组的年龄、性别、体质量指数、患眼、吸烟史、饮酒史、高血压史、高脂血症史、冠心病史比较差异均无统计学意义(P>0.05),两组病程、治疗前BCVA、治疗前眼压、糖尿病史比较差异均有统计学意义(P<0.05)。病程短、治疗前眼压低均是影响激光光凝术后RNV患者视功能的保护因素,而治疗前BCVA低、有糖尿病史均是影响患者术后视功能的危险因素。结论 病程、治疗前眼压、治疗前BCVA、糖尿病史均会影响激光光凝术后RNV患者视功能。临床应给予重视,完善早期评估,对存在以上因素的患者进行及时干预,以改善其术后视功能。

【Abstract】 Objective To explore the influence factors of retinal neovascularization(RNV) on visual function after laser photocoagulation.Methods A total of 120 RNV patients who received laser photocoagulation in our hospital were included in the study from January 2021 to July2023. Best corrected visual acuity(BCVA) were measured at 1 day before treatment and 3 months after surgery to assess the visual function, and pa-tients were divided into poor and good visual function groups based on BCVA at 3 months after surgery. The clinical data of two groups were com-pared, and the related factors were analyzed by multiple factors. Results The BCVA at 3 months after surgery was apparently higher than that at 1day before treatment(P<0.05). There were no statistically significant differences in age, gender, body mass index, affected eye, history of smoking,drinking, hypertension, hyperlipidemia and coronary heart disease between the poor and good visual function groups( P>0.05), while there were statistically significant differences in disease course, BCVA and intraocular pressure before treatment, and history of diabetes between the poor and good visual function groups(P<0.05). Short disease course and low intraocular pressure before treatment were protective factors for the visual function of RNV patients after laser photocoagulation, while low BCVA before treatment and history of diabetes were risk factors for the visual function of patients. Conclusion The disease course, BCVA and intraocular pressure before treatment, and history of diabetes all affected the visual function of RNV patients after laser photocoagulation. Clinical attention should be paid to improve early assessment and timely intervention for patients with the above factors to improve their visual function after surgery.
