家庭功能与网络成瘾对中学生抑郁症状交互作用

【摘要】 目的 分析家庭功能和网络成瘾及其交互作用与中学生抑郁症状的相关性,为中学生抑郁的预防提供理论依据。方法 本研究采用分层整群随机抽样,2020年10—11月在湖北省孝昌县抽取3 209名中学生进行问卷调查。问卷包括自编基本情况调查表、流调中心用抑郁量表(the center for epidemiologic studies depression scale, CES-D)、网络成瘾诊断量表(the internet addiction test,IAT)、家庭关怀度指数量表(family adaptation partnership growth affection and resolve index, APGAR)。采用多因素logistic回归模型分析家庭功能和网络成瘾与中学生抑郁症状的相关性及其交互作用。结果 中学生抑郁症状、网络成瘾和家庭功能障碍分别检出838名(26.11%)、585名(18.23%)、1 796名(55.97%)。Logistic回归分析结果显示,家庭功能障碍(OR=2.91,95%CI:2.39~3.55)、网络成瘾(OR=4.46,95%CI:3.59~5.53)是中学生出现抑郁症状的危险因素(P<0.01)。交互作用结果显示,家庭功能与网络成瘾对抑郁症状的检出具有相加交互作用,超额相对危险度、归因比、交互作用指数分别为6.40 (95%CI:2.92~9.90)、0.50(95%CI:0.33~0.66)和2.16 (95%CI:1.49~3.12)。结论 在对有家庭功能障碍的中学生抑郁防治过程中,应重点关注网络成瘾问题,通过改善网络成瘾问题减轻家庭功能障碍对抑郁的不良影响。

【Abstract】 Objective To analyze the associations of family functioning and internet addiction, and that of their interaction, with depressive symptoms of middle school students, so as to provide theoretical basis for depression prevention in this population. Methods A questionnaire survey was conducted among 3 209 middle school students selected using stratified cluster random sampling in Xiaochang County, Hubei Province, from October to November 2020. The questionnaire included a self-designed basic information questionnaire, the center for epidemiologic studies depression scale(CES-D), the internet addiction test(IAT), and the family adaptation partnership growth affection and resolve(APGAR) index. Multivariable logistic regression was used to analyze the association of family functioning and internet addiction with depressive symptoms in middle school students,and the interaction between the two factors. Results The depressive symptoms, internet addiction, and family dysfunction among the included middle school students were 838 cases(26. 11%), 585 cases(18. 23%), and 1 796 cases(55. 97%). Logistic regression analysis showed that family dysfunction(OR=2. 91, 95%CI: 2. 39-3. 55) and internet addiction(OR=4. 46, 95%CI: 3. 59-5. 53) were risk factors for depressive symptoms in middle school students(P<0. 01). Interaction analysis results showed that relative excess risk of interaction(RERI), attributable proportion(AP), and synergy index(SI) were 6. 40(95%CI: 2. 92-9. 90), 0. 50(95%CI:0. 33-0. 66), and 2. 16(95%CI: 1. 49-3. 12), respectively. Conclusions In depression prevention and treatment for middle school students from dysfunctional families, measures addressing internet addiction should also be considered so as to mitigate the negative impact of family dysfunction on depression.
