微创表面活性物质注射技术联合无创通气在早产儿轻中度新生儿呼吸窘迫综合征中的应用
Application of less invasive surfactant administration technique combined with noninvasive positive pressure ventilation in mild to moderate neonatal respiratory distress syndrome of premature infants
【摘要】 目的 探讨微创表面活性物质注射(LISA)技术联合无创通气(NIPPV)在早产儿轻中度新生儿呼吸窘迫综合征(NRDS)中的应用效果。方法 选取2022年3月至2023年3月江西省妇幼保健院收治的80例NRDS患儿为研究对象,采用随机数字表法分为常规组与研究组,每组各40例。其中常规组传统技术联合NIPPV进行治疗,研究组使用LISA技术联合NIPPV进行治疗,比较两组患儿疗效、血气指标[p H值、血氧分压(Pa O2)、二氧化碳分压(Pa CO2)及氧合指数(OI)]、临床指标[再次肺表面活性物质(PS)治疗例数、72 h内机械通气例数、机械通气时间、住院时间]及并发症发生情况。结果 研究组总有效率(97.50%)高于常规组(80.00%),差异有统计学意义(P<0.05);治疗后,研究组p H、Pa O2及OI值均高于常规组,Pa CO2低于常规组,差异有统计学意义(P<0.05)。两组患儿再次PS发生率比较,差异无统计学意义(P>0.05)。研究组72 h内机械通气发生率低于常规组,机械通气时间及住院时间短于常规组,差异有统计学意义(P<0.05)。研究组并发症总发生率(5.00%)低于常规组(22.50%),差异有统计学意义(P<0.05)。结论 在治疗早产儿轻中度NRDS患儿方面,LISA技术疗效确切,相较于传统技术其改善患儿血气指标的效果更加明显,同时可显著减少患儿机械通气次数及时间,缩短住院时间,并显著减少并发症的发生。
【Abstract】 Objective To investigate the effect of less invasive surfactant administration(LISA) technique combined with noninvasive positive pressure ventilation(NIPPV) in mild to moderate neonatal respiratory distress syndrome(NRDS) of premature infants.Methods A total of 80 children with NRDS treated in Jiangxi Maternal and Child Health Hospital from March 2022 to March 2023 were selected as the research objects,and they were divided into routine group and study group according to random number table method,with 40 cases in each group.Among them,the routine group was treated with traditional technique combined with NIPPV,and the study group was treated with LISA technique combined with NIPPY.The therapeutic effect,blood gas indexes(pH value,partial pressure of oxygen in arterial blood [PaO2],partial pressure of carbon dioxide [PaCO2] and oxygenation index [OI]),clinical indexes(number of repeated pulmonary surfactant [PS]treatment cases,number of mechanical ventilation cases within 72 h,mechanical ventilation time,hospital stay) and complications were compared between the two groups.Results The total effective rate of the study group(97.50%) was higher than that of the routine group(80.00%),and the difference was statistically significant(P<0.05).After treatment,pH,PaO2 and OI values in the study group were higher than those in the routine group,and PaCO2 was lower than that in the routine group,with statistical significances(P<0.05).There was no statistical significance in the incidence of repeated PS recurrence between the two groups(P>0.05).The incidence of mechanical ventilation within 72 h in the study group was lower than that in the routine group,the duration of mechanical ventilation and hospital stay were shorter than those in the routine group,and the differences were statistically significant(P<0.05).The total incidence of complications in the study group(5.00%) was lower than that in the routine group(22.50%),and the difference was statistically significant(P<0.05).Conclusion In the treatment of premature infants with mild to moderate NRDS,LISA technique is effective,and compared with traditional technique,it has more obvious effect on improving blood gas indexes in children,and can significantly reduce the number and shorten time of mechanical ventilation in children,shorten the length of hospital stay,and significantly reduce the occurrence of complications.
